糖尿病是一种血糖水平异常增高的内分泌系统疾病,是全球范围内最常见,增长最快的疾病之一。糖尿病及其并发症的流行极大地加重了全世界的残疾负担和死亡率,已经对全球健康构成了重大威胁。因此,研究糖尿病及其并发症的发病机制,发展新的预防和治疗药物成为当务之急。灵芝,是我国珍贵的传统中药材,在传统中医领域的应用源远流长。灵芝多糖是灵芝的主要有效成分之一,具有多种药理作用,包括抗肿瘤、提高免疫、抗肥胖及降血糖等作用。目前,有关灵芝多糖在糖尿病及其并发症中的综述报道十分缺乏。本文将首先对灵芝多糖在糖尿病中的作用及分子机制进行综述。然后将重点对灵芝多糖在改善糖尿病并发症中的研究进展进行综述,包括糖尿病肾病、糖尿病相关心脏疾病、糖尿病神经病变及伤口不愈症,并阐明其可能的分子作用机制。这项综述将从多方面为灵芝多糖抗糖尿病及其并发症的基础研究及临床应用提供理论依据。

Diabetes is an endocrine system disease with abnormally high blood sugar levels. It is one of the most common and fastest-growing diseases in the world. The prevalence of diabetes and its complications has greatly increased the burden of disability and mortality worldwide, and has posed a major threat to global health. Therefore, it is urgent to study the pathogenesis of diabetes and its complications and to develop new drugs for prevention and treatment. Ganoderma lucidum is a precious traditional Chinese medicine (TCM) in China, which has a long history of application in TCM. Ganoderma lucidum polysaccharide (GLP) is one of the main bioactive compounds of Ganoderma lucidum. It has a variety of pharmacological effects, including anti-tumor, immune regulation, anti-obesity and blood glucose lowering effects. At present, the review about GLP in diabetes and its complications is very scarce. In this review, we will first review the anti-diabetic role and the underlying molecular mechanisms of GLP from the literature. We will then focus on reviewing the progress of GLP in improving diabetes complications, including diabetic kidney disease, diabetic heart disease, diabetic neuropathy and refractory wounds, and clarifying its possible molecular mechanisms. This review will provide theoretical basis for the laboratory studies and clinical application of GLP in anti-diabetes and its complications from a broad aspect.
