冠状动脉粥样硬化性心脏病发病率高、疾病负担沉重。西医常规治疗虽可有效控制危险因素、重建冠脉血运,但难以解决微循环障碍、残余炎症及整体功能调节缺失等问题。中医认为冠心病属“胸痹”“心痛”范畴,以“气虚血瘀”为核心病机,与现代医学“血管损伤-炎症免疫-代谢失衡”的病理网络深度契合。芪参益气滴丸由黄芪、丹参、三七、降香配伍而成,紧扣益气活血治则,兼具多靶点系统调控优势。以系统调控与精准干预为主线,首先厘清气虚血瘀病机的科学内涵及其与现代病理网络的关联,进而通过“分子事件-信号通路-生物效应”3层级的机制挖掘,系统阐释芪参益气滴丸抗炎、抗氧化、抗凋亡、促血管新生及改善能量代谢的协同调控网络。通过临床证据分层评价体系,总结该药在症状改善、心功能提升、远期预后及特殊人群中的应用价值,并提出未来研究可依托多组学技术、真实世界研究等方法,深化芪参益气滴丸作用机制的探索,开展高质量临床循证研究,建立统一的疗效评价体系,优化个体化用药方案,进一步推动该药物在冠心病中西医结合诊疗中的规范化应用。

Coronary heart disease (CHD) is characterized by a high incidence rate and a substantial disease burden. Although conventional Western medicine can effectively manage cardiovascular risk factors and achieve coronary revascularization, it often fails to adequately address microcirculation disturbance, residual inflammatory risk, and the lack of holistic systemic regulation. In traditional Chinese medicine (TCM), CHD falls under the categories of “Xiongbi” (chest obstruction) and “Xintong” (cardiac pain), with “qi deficiency and blood stasis” identified as the core pathogenesis. This concept aligns closely with the modern pathological network of “vascular injury-inflammatory immune response-metabolic dysregulation”. Qishen Yiqi Dripping Pills (芪参益气滴丸, QSYQ), a Chinese patent medicine composed of Huangqi (Astragali Radix), Danshen (Salviae Miltiorrhizae Radix et Rhizoma), Sanqi (Notoginseng Radix et Rhizoma), and Jiangxiang (Dalbergiae Odoriferae Lignum), strictly adheres to the therapeutic principle of supplementing qi and activating blood, offering distinct advantages in multi-target systemic regulation. Centered on the theme of systemic regulation and precise intervention, this review first elucidates the scientific connotation of qi deficiency and blood stasis and maps its correlation with modern pathological networks. It then establishes a three-tiered mechanistic framework spanning “molecular events-signaling pathways-biological effects” to systematically decode the synergistic regulatory networks of QSYQ in anti-inflammation, anti-oxidation, anti-apoptosis, angiogenesis promotion, and energy metabolism optimization. Furthermore, a hierarchical clinical evidence evaluation system is constructed to summarize the therapeutic value of QSYQ in alleviating symptoms, improving cardiac function, enhancing long-term prognosis, and managing special patient populations. Future research should leverage multi-omics technologies and real-world evidence studies to further elucidate the underlying mechanisms of action. Additionally, high-quality evidence-based studies should be conducted to standardize evaluation criteria and optimize individualized medication regimens, ultimately promoting the standardized clinical application of QSYQ in the integrative traditional Chinese and Western medicine treatment of coronary heart disease.
