[摘要]
目的 对比注射用益气复脉、生脉注射液、参麦注射液和参附注射液在改善大鼠慢性心力衰竭(chronic heart failure,CHF)的药效。方法 选取10只Wistar大鼠作为对照组,90只大鼠建立CHF模型;CHF大鼠随机分成模型组、注射用益气复脉(0.46 g/kg)组、生脉注射液(5.4 mL/kg)组、参麦注射液(5.4 mL/kg)组、参附注射液组(5.4 mL/kg)和卡托普利(3.67 mg/kg)组,每组15只,给予药物干预2周后,通过多普勒超声检测4种中药注射剂对CHF大鼠心脏左室短轴缩短率(left ventricular fractional shortening,LVFS)、左室射血分数(left ventricular ejection fraction,LVEF)、二尖瓣血流频谱E峰/A峰(E/A)等心功能指标的影响;采用ELISA试剂盒检测各组大鼠血清中心钠肽(atrial natriuretic peptide,ANP)、脑钠肽(brain natriuretic peptide,BNP)、和肽素(copeptin,CCP)、肌酸激酶同工酶(creatine kinase isoenzyme-MB,CK-MB)、肌钙蛋白(troponin,Tn)、乳酸脱氢酶(lactate dehydrogenase,LDH)、超敏C反应蛋白(hypersensitive C-reactive protein,CRP)、白细胞介素-6(interleukin-6,IL-6)等水平的变化;采用苏木素-伊红(HE)和Masson染色法观察4种中药注射剂对CHF大鼠心脏组织病理变化的影响。结果 与对照组比较,模型组大鼠心功能指标LVEF、LVFS和E/A均显著降低(P<0.01),血清中ANP、BNP、CCP、Tn、CK-MB、IL-6、CRP和LDH水平显著升高(P<0.01),大鼠心肌细胞病变、纤维组织增生;与模型组比较,各给药组大鼠LVEF、LVFS和E/A均显著升高(P<0.01),注射用益气复脉组大鼠血清中ANP、BNP、CCP、Tn、CK-MB、IL-6、CRP和LDH水平均显著降低(P<0.05、0.01),参麦注射液组和参附注射液组大鼠血清中BNP、CCP、IL-6和CRP水平显著降低(P<0.05、0.01),各给药组大鼠心肌组织损伤及纤维化得到明显改善。结论 注射用益气复脉、生脉注射液、参麦注射液和参附注射液能够通过提高CHF大鼠心脏的射血能力和收缩能力改善心功能,通过改善心脏组织形态的变化及心肌纤维化缓解心室重构,通过降低血液中心肌损伤指标水平缓解大鼠心力衰竭,其中注射用益气复脉疗效最佳。

Objective To compare the effects of Yiqi Fumai Injection (注射用益气复脉), Shengmai Injection (生脉注射液), Shenmai Injection (参麦注射液) and Shenfu Injection (参附注射液) on improving chronic heart failure (CHF) in rats. Methods Ten Wistar rats were selected as control group, and 90 rats were established for CHF model; CHF rats were randomly divided into model group, Yiqi Fumai Injection (0.46 g/kg) group, Shengmai Injection (5.4 mL/kg) group, Shenmai Injection (5.4 mL/kg) group, Shenfu Injection (5.4 mL/kg) group and captopril (3.67 mg/kg) group, 15 rats in each group. After 2 weeks of drug intervention, the effects of four Chinese medicine injections on cardiac function indexes such as left ventricular fractional shortening (LVFS), left ventricular ejection fraction (LVEF), and mitral valve blood flow spectrum E peak/A peak (E/A) of CHF rats were detected by Doppler ultrasound; ELISA was used to detect the levels of atrial natriuretic peptide (ANP), brain natriuretic peptide (BNP), copeptin (CCP), creatine kinase isoenzyme (CK-MB), troponin (Tn), lactate dehydrogenase (LDH), hypersensitive C-reactive protein (CRP), interleukin-6 (IL-6) in serum of rats; Hematoxylin-eosin (HE) and Masson staining methods were used to observe the effects of four traditional Chinese medicine injections on pathological changes of heart tissue in CHF rats. Results Compared with control group, the cardiac function indexes LVEF, LVFS and E/A of model group were significantly reduced (P < 0.01), and the levels of ANP, BNP, CCP, Tn, CK-MB, IL-6, CRP and LDH in serum were significantly increased (P < 0.01), cardiomyocyte was pathological changed and fibrous tissue were hyperplasia. Compared with model group, LVEF, LVFS and E/A of rats in each administration group were significantly increased (P < 0.01), the levels of ANP, BNP, CCP, Tn, CK-MB, IL-6, CRP and LDH in serum of rats in Yiqi Fumai Injection group were significantly reduced (P < 0.05, 0.01), levels of BNP, CCP, IL-6 and CRP in serum of rats in Shenmai Injection group and Shenfu Injection group were significantly reduced (P < 0.05, 0.01), myocardial tissue damage and fibrosis of rats in each administration group were significantly improved. Conclusion Yiqi Fumai Injection, Shengmai Injection, Shenmai Injection and Shenfu Injection can improve heart function by improving the ejection capacity and contractility of heart of CHF rats, alleviate ventricular remodeling by improving the changes of tissue morphology and myocardial fibrosis, and relieve the degree of heart failure in rats by reducing the content of myocardial damage index in blood, and Yiqi Fumai Injection has the best effect.
