[摘要]
目的 探讨丹红注射液联合磺达肝癸钠对肺栓塞患者免疫学指标及凝血-纤溶系统的影响。方法 选择2014年8月—2018年8月在喀什地区第二人民医院确诊的80例肺栓塞患者作为研究对象。随机将患者分为对照组和观察组,每组各40例。对照组患者皮下注射磺达肝癸钠注射液,2.5 mg/次,1次/d,第4天起联合应用华法林抗凝5 d,并通过监测国际标准化比值(INR)来调整华法林用量,之后停用磺达肝癸钠并持续应用华法林抗凝。观察组在对照组治疗的基础上静脉滴注丹红注射液,将20 mL丹红注射液加入到氯化钠溶液250 mL中,1次/d。两组均连续治疗2周。观察两组患者的临床疗效,同时比较两组治疗前后的免疫学指标、凝血-纤溶系统指标水平。结果 治疗后,观察组总有效率为90.00%,显著高于对照组的72.50%(P<0.05)。与治疗前相比,两组各免疫学指标均明显改善(P<0.05),且观察组优于对照组(P<0.05)。治疗后,两组D-二聚体(D-D)、纤维蛋白原降解产物(FDP)、纤溶酶原激活物抑制剂(PAI-I)与组织型纤溶酶原激活物(tPA)活性均显著降低,凝血酶原时间(PT)与血小板计数(PLT)水平显著升高(P<0.05),且观察组凝血-纤溶系统指标水平显著优于对照组(P<0.05)。结论 丹红注射液联合磺达肝癸钠可有效改善患者的免疫功能及凝血-纤溶系统,疗效安全显著,对促进肺栓塞患者病情康复具有积极意义。

Objective To explore the effect of Danhong Injection combined with fondaparinux sodium on immunological indicators and coagulation-fibrinolysis system in patients with pulmonary embolism. Methods Patients (80 cases) with pulmonary embolism in the Second People's Hospital of Kashgar from August 2014 to August 2018 were selected as the study subjects. Patients were randomly divided into control group and observation group, with 40 patients in each group. Patients in the control group were subcutaneously injected with Fondaparinux Sodium Injection, 2.5 mg/time, once daily, combined with warfarin anticoagulation for 5 days from the 4th day, and the dosage of warfarin was adjusted by monitoring the INR. After that, Fondaparinux Sodium Injection was stopped and warfarin anticoagulation was continued. Patients in the observation group were iv administered with Danhong Injection on the basis of control group, and 20 mL Danhong Injection was added to 250 mL sodium chloride solution, once daily. Both groups were treated continuously for 2 weeks. The clinical efficacy in two groups was observed, and the indexes of immunology and coagulation-fibrinolysis system were compared before and after treatment. Results After treatment, the total effective rate in the observation group was 90.00%, which was significantly higher than 72.50% in the control group (P<0.05).Compared with before treatment, the immunological indexes in two groups were significantly improved (P<0.05), and the indexes in the observation group was better than those in control group (P<0.05). Compared with before treatment, the levels of D-D, FDP, PAI-I, and tPA in two groups were significantly decreased after treatment, and the levels of PT and PLT were significantly increased (P<0.05). Moreover, the indicators of coagulation and fibrinolysis in the observation group were significantly better than those in the control group (P<0.05). Conclusion Danhong Injection combined with fondaparinux sodium can effectively improve the immune function and the coagulation and fibrinolysis system of patients, with a safe and significant effect, and has positive significance in promoting the rehabilitation of patients with pulmonary embolism.
