[摘要]
目的 系统评价槐杞黄颗粒治疗小儿反复呼吸道感染的疗效及安全性。方法 计算机检索Cochrane Library、Embase、Pubmed、中国生物医学文献数据库(CBM)、中国学术期刊全文数据库(CNKI)、维普中文期刊全文数据库(VIP)和万方数据库等数据库,搜集槐杞黄颗粒治疗小儿反复呼吸道感染的随机对照试验(RCT),检索时限均从建库至2019年9月。对符合纳入标准的研究进行质量评价和资料提取后,应用RevMan 5.2软件进行Meta-分析。结果 最终纳入16项RCTs,计1 624例患儿。Meta-分析结果显示:与对照组相比,槐杞黄颗粒组能显著提高小儿反复呼吸道感染的临床有效率[OR=6.23,95% CI(4.32,8.99),P<0.000 01],治疗前后血清免疫球蛋白IgA水平[WMD=0.59,95% CI(0.20,0.98),P=0.003]、IgG水平[WMD=1.32,95% CI(0.99,1.64),P<0.000 01]、IgM水平[WMD=0.16,95% CI(0.07,0.25),P=0.00 03]均优于对照组,两组差异比较具有统计学意义。纳入研究中槐杞黄颗粒的不良反应表现轻微,均无严重不良反应发生。结论 槐杞黄颗粒治疗小儿反复呼吸道感染的疗效总体优于常规治疗,可改善各项免疫功能指标。鉴于本系统评价纳入研究的样本量较少,仍需开展严格的、大样本、多中心的随机对照试验加以验证。

[Abstract]
Objective To systematically evaluate the efficacy and safety of Huaiqihuang Granules for treating recurrent respiratory tract infection in children. Method Searched the Cochrane Library, EMbase, PubMed, CBM, CNKI and Wanfang Data from inception to 2019 September. All randomized controlled trials (RCTs) of Huaiqihuang Granules for recurrent respiratory tract infection in children were collected. According to the inclusion and exclusion criteria, literature were extracted, and the methodological quality of included studies was also assessed. Meta-analysis was performed by RevMan 5.2 software. Results A total of 16 RCTs involving 1 624 cases were included. The results of Meta-analysis showed that compared with the control group, Huaiqihuang Granules can significantly improve the clinical efficacy[OR=6.23, 95%CI (4.32, 8.99), P<0.000 01], and increase IgA levels[WMD=0.59, 95%CI (0.20, 0.98), P=0.003], IgG levels[WMD=1.32, 95%CI (0.99, 1.64), P<0.000 01], and IgM levels[WMD=0.16, 95%CI (0.07, 0.25), P=0.000 3] after treatment with significant differences. And the adverse drug reactions of Huaiqihuang Granules were slight with no serious occurrence. Conclusion Actual clinical evidence indicated that efficacy of Huaiqihuang Granules was superior than conventional therapy in the treatment of children with recurrent respiratory tract infection. However, due to the small size of included RCTs in the study, more strictly designed, large scale and multi-center RCTs were required for further validation.
