[摘要]
目的 探究甘草酸二胺对腹膜透析患者的腹膜功能和炎症因子的影响。方法 将2017年1月—2018年2月在珠海市人民医院接受腹膜透析治疗的114例患者随机分成对照组和观察组,每组各57例。对照组采用持续非卧床腹膜透析(PD),选用1.5% PD液,2 000 mL/次,3~4次/d。观察组在对照组治疗的基础上口服甘草酸二铵胶囊,150 mg/次,3次/d。两组干预时间均为6个月。比较两组患者治疗前后腹膜功能以及血炎症因子水平的变化。结果 观察组的超滤量和肌酐和葡萄糖的比值(D/P)和高于对照组,观察组的0 h和4 h肌酐比值(D0/D4)低于对照组,差异均有显著性(P<0.05)。治疗后,两组血浆与PD液中结缔组织生长因子(CTGF)、转化生长因子(TGF-β1)和血管内皮生长因子(VEGF)水平较治疗前均显著升高(P<0.05);治疗后观察组的血浆和腹膜透析液TGF-β1、VEGF、CTGF均低于对照组,差异均有显著性(P<0.05)。观察组治疗后白介素-1(IL-1)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)均显著低于治疗前(P<0.05);对照组治疗后IL-6和TNF-α显著低于治疗前(P<0.05);治疗后观察组的IL-1和TNF-α显著低于对照组(P<0.05)。两组治疗后尿素氮(BUN)、肌酐(Cr)水平与治疗前相比较均显著下降(P<0.05);且治疗后观察组的BUN、Cr水平显著低于对照组(P<0.05);两组患者治疗前后K+浓度无明显变化。结论 甘草酸二胺能够明显改善患者腹膜功能,延缓腹膜纤维化,降低炎症因子的表达水平。

Objective To explore the effect of diammonium glycyrrhizinate on peritoneal function and inflammatory factors in peritoneal dialysis patients. Methods Patients (114 cases) who received peritoneal dialysis in Zhuhai People's Hospital from January 2017 to February 2018 were randomly divided into control group and observation group, and each group had 57 cases. Patients in the control group was treated with continuous ambulatory peritoneal dialysis with 1.5% PD solution, 2 000 mL/time, three or four times daily. Patients in the observation group were po administered with Diammonium Glycyrrhizinate Capsules on the basis of control group, 150 mg/time, three times daily. Patients in two groups were treated for 6 months. The changes of peritoneal function and serum inflammatory factors in two groups before and after treatment were compared. Results Ultrafiltration and D/P in the observation group were higher than those in the control group, while D0/D4 in the observation were lower than those in the control group (P<0.05). After treatment, the levels of CTGF, TGF-β1 and VEGF in plasma and PD fluid in two groups were significantly higher than those before treatment (P<0.05). After treatment, the levels of TGF-β1, VEGF and CTGF in plasma and PD fluid in observation group were significantly lower than those of the control group (P<0.05). The levels of IL-1, IL-6, TNF-α in the observation group were significantly lower than those before treatment (P<0.05), the levels of IL-6 and TNF-α in the control group were significantly lower than those before treatment (P<0.05), and the levels of IL-1 and TNF-α in the observation group were significantly lower than those in the control group (P<0.05). After treatment, BUN and Cr levels in two groups were significantly lower than those before treatment (P<0.05), and BUN and Cr levels in the observation group were significantly lower than those in the control group (P<0.05). There was no significant change in K+ concentration in two groups before and after treatment.Conclusion Diammonium glycyrrhizinate can significantly improve the peritoneal function, delay peritoneal fibrosis, and reduce the expression of inflammatory factors.
