目的 观察寒喘祖帕颗粒联合盐酸班布特罗片治疗咳嗽变异性哮喘的临床研究。方法 选择南通市第六人民医院2022年4月—2024年4月收治的98例咳嗽变异性哮喘患者，将所有患者按照随机数字表法分为对照组（49例）和治疗组（49例）。对照组每晚睡前口服盐酸班布特罗片，10 mg/次，根据临床效果，在用药1～2周后可增加到20 mg/次。治疗组接受盐酸班布特罗片基础上口服寒喘祖帕颗粒，12 g/次，2次/d。两组均治疗4周。对比两组的临床疗效、临床症状、肺功能指标、血清炎症因子指标和免疫功能指标。结果 对照组的总有效率为69.39%，治疗组的总有效率为91.84%，组间比较差异具有显著性（P＜0.05）。与对照组相比，治疗组的哮喘发生次数减少，咳嗽消失时间、咳嗽缓解时间更短（P＜0.05）。治疗后两组第一秒用力呼气容积（FEV1）、用力肺活量（FVC）、最大呼气流速峰值（PEF）升高（P＜0.05），且治疗组的FEV1、FVC、PEF高于对照组（P＜0.05）。治疗后两组血清白细胞介素-6（IL-6）、单核细胞趋化蛋白-1（MCP1）、肿瘤坏死因子-α（TNF-α）、巨噬细胞炎症蛋白-1α（MIP-1α）水平下降（P＜0.05），且治疗组患者血清IL-6、MCP1、TNF-α、MIP-1α水平低于对照组（P＜0.05）。治疗后两组CD3⁺、CD4⁺、CD4⁺/CD8⁺升高，CD8⁺下降（P＜0.05），且治疗组的CD3⁺、CD4⁺、CD4⁺/CD8⁺高于对照组，CD8⁺低于对照组（P＜0.05）。结论 咳嗽变异性哮喘使用寒喘祖帕颗粒、盐酸班布特罗片联合治疗，可改善患者临床症状，提高肺功能，控制气道炎症反应，提升机体免疫功能。

Objective To observe the clinical study of Hanchuan Zupa Granules combine with Bambuterol Hydrochloride Tablets in treatment of cough variant asthma. Methods 98 Patients with cough variant asthma admitted to Nantong Sixth People's Hospital from April 2022 to April 2024 were divided into control group (49 cases) and treatment group (49 cases) with random number table method. The control group took 10 mg Bambuterol Hydrochloride Tablets orally before bedtime every night, and according to clinical efficacy, it could be increased to 20 mg after 1 — 2 weeks of medication. The treatment group receives Bambuterol Hydrochloride Tablets in addition to oral Hanchuan Zupa Granules, 12 g/time, twice daily. Two groups were treated for 4 weeks. The clinical efficacy, clinical symptoms, lung function indicators, serum inflammatory factor indicators, and immune function indicators were compared between two groups. Results The total effective rate of the control group was 69.39%, the total effective rate of the treatment group was 91.84%, and the difference was significant between the groups (P < 0.05). Compared with the control group, the treatment group had a decreased frequency of asthma, shorter cough disappearance time, and shorter cough relief time (P < 0.05). After treatment, the FEV1, FVC, and PEF of two groups were increased (P < 0.05), and the FEV1, FVC, and PEF of the treatment group were higher than those of the control group (P < 0.05). After treatment, the serum levels of IL-6, MCP1, TNF-α, and MIP-1α were significantly decreased in two groups (P < 0.05), and the serum levels of IL-6, MCP1, TNF-α, and MIP-1α in the treatment group were lower than those in the control group (P < 0.05). After treatment, the levels of CD3⁺, CD4⁺, and CD4⁺/CD8⁺ were increased in two groups, while CD8⁺ were decreased (P < 0.05). The levels of CD3⁺, CD4⁺, and CD4⁺/CD8⁺ in the treatment group were higher than those in the control group, while CD8⁺was lower than those in the control group (P < 0.05). Conclusion The combination of Hanchuan Zupa Granules and Bambuterol Hydrochloride Tablets can improve clinical symptoms in the treatment of cough variant asthma, enhance lung function, control airway inflammation, and improve immune function.
