目的 探讨银杏叶提取物注射液联合桂哌齐特治疗突发性耳聋患者的临床疗效。方法 选取2017年1月—2018年9月新疆维吾尔自治区人民医院收治的突发性耳聋患者130例，随机分为对照组和治疗组，每组各65例。对照组静脉滴注马来酸桂哌齐特注射液，80 mg加入生理盐水250 mL，1次/d。治疗组在对照组的基础上静脉滴注银杏叶提取物注射液，105 mg加入生理盐水250 mL，1次/d。两组均治疗2周。观察两组患者临床疗效，同时比较治疗前后两组患者伴随症状和不同频率听阈改善情况以及血液流变学、免疫功能和血清学指标。结果 治疗后，对照组总有效率为86.15%，显著低于治疗组的95.38%，两组比较差异均具有统计学意义（P<0.05）。治疗后，治疗组眩晕、耳鸣、耳闷等伴随症状改善率均明显高于对照组，两组比较差异具有统计学意义（P<0.05）。治疗后，治疗组各频率平均听力阈值改善幅度明显高于对照组，两组比较差异具有统计学意义（P<0.05）。治疗后，两组全血高切黏度（WBHSV）、中切黏度（WBMSV）、低切黏度（WBLSV）、血浆比黏度、纤维蛋白原（FIB）水平均明显下降（P<0.05），且治疗组上述血液流变学指标明显低于对照组（P<0.05）。治疗后，两组CD3⁺、CD4⁺、CD4⁺/CD8⁺水平明显升高（P<0.05），CD8⁺明显降低（P<0.05），且治疗后治疗组免疫功能明显好于对照组（P<0.05）。治疗后，两组肿瘤坏死因子-α（TNF-α）、白细胞介素-8（IL-8）水平明显下降（P<0.05），基质金属蛋白酶-9（MMP-9）水平明显上升（P<0.05），且治疗后治疗组IL-8、TNF-α和MMP-9水平明显好于对照组（P<0.05）。结论 银杏叶提取物注射液联合桂哌齐特可显著改善患者听力状况和伴随症状，具有一定的临床推广应用价值。

Objective To investigate the clinical efficacy of Extract of Ginkgo Biloba Leaves Injection combined with cinepazide in treatment of sudden deafness. Methods Patients (130 cases) with sudden deafness in People's Hospital of Xinjiang Uygur Autonomous Region from January 2017 to September 2018 were randomly divided into control and treatment groups, and each group had 65 cases. Patients in the control group were iv administered with Cinepazide Maleate Injection, 80 mg added into 250 mL normal saline, once daily. Patients in the treatment group were iv administered with Extract of Ginkgo Biloba Leaves Injection on the basis of the control group, 105 mg added into 250 mL normal saline, once daily. Patients in two groups were treated for 2 weeks. After treatment, the clinical efficacy was evaluated, and the improvement of concomitant symptoms and hearing threshold, and the hemorheological, immune function, and serological indicators in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control group was 86.15%, which were significantly lower than 95.38% in the treatment group, and there were differences between two groups (P < 0.05). After treatment, the improvement rate of concomitant symptoms such as vertigo, tinnitus, and tightness in the treatment group was significantly higher than that in the control group, with significant difference between two groups (P < 0.05). After treatment, each frequency of average hearing threshold in the treatment group was significantly higher than that in the control group, with significant difference between two groups (P < 0.05). After treatment, the WBHSV, WBMSV, WBLSV, plasma viscosity and FIB levels in two groups were significantly decreased (P < 0.05), and the hemorheological indexes in the treatment group were significantly lower than those in the control group (P < 0.05). After treatment, the CD3⁺, CD4⁺, and CD4⁺/CD8⁺ levels in two groups were significantly increased (P < 0.05), but CD8⁺ level was significantly decreased (P < 0.05), and the immune function in the treatment group was significantly better than those in the control group (P < 0.05). After treatment, theIL-8 and TNF-α levels in two groups were significantly decreased (P < 0.05), but MMP-9 levels were significantly increased (P < 0.05), and the IL-8, TNF-α, and MMP-9 levels in the treatment group were significantly better than those in the control group (P < 0.05). Conclusion Extract of ginkgo biloba leaves combined with cinepazide can improve the hearing condition and concomitant symptoms, which has a certain clinical application value.
