目的 探讨银杏二萜内酯葡胺联合奥扎格雷治疗急性脑梗死的安全性与有效性。方法 选取2016年6月-2017年6月于河南中医药大学第一附属医院诊治的急性脑梗死患者121例，按照入院顺序分成对照组（60例）和治疗组（61例）。对照组患者静脉滴注注射用奥扎格雷钠，80 mg溶解于500 mL生理盐水，2次/d；治疗组在对照组用药基础上静脉滴注银杏二萜内酯葡胺注射液，5 mL加入250 mL生理盐水，1次/d。两组患者均连续用药14 d。观察两组患者临床疗效，同时比较治疗前后两组患者Barthel指数（BI）、美国国立卫生研究院卒中量表（NIHSS）和中风患者生存质量量表（QOLISP）评分及血清脂联素、神经元特异性烯醇化酶和过氧化氢酶水平及不良反应情况。结果 治疗后，对照组临床有效率为81.67%，显著低于治疗组的95.08%，两组比较差异具有统计学意义（P<0.05）。治疗后，两组患者BI、NIHSS和QOLISP评分显著升高（P<0.05），且治疗组BI评分、NIHSS评分和QOLISP评分明显高于对照组（P<0.05）。治疗后，两组患者血清脂联素和过氧化氢酶水平显著升高（P<0.05），血清神经元特异性烯醇化酶水平显著降低（P<0.05），且治疗组上述指标水平明显好于对照组（P<0.05）。治疗期间，治疗组患者药物不良反应发生率为3.28%，显著低于对照组15.00%，两组比较差异具有统计学意义（P<0.05）。结论 银杏二萜内酯葡胺联合奥扎格雷治疗急性脑梗死临床效果好、安全性高，能够显著改善患者的生活能力与生存质量，具有一定的临床推广应用价值。

Objective To investigate the safety and efficacy of Ginkgo Biloba Diterpene Lactone Glumine Injection combined with ozagrel in treatment of acute cerebral infarction. Methods Patients (121 cases) with acute cerebral infarction in the First Affiliated Hospital of Henan University of TCM from June 2016 to June 2017 were divided into control (60 cases) and treatment (61 cases) groups based on admission order. Patients in the control group were iv administered with Ozagrel Sodium for injection, 80 mg dissolved in normal saline 500 mL, twice daily. Patients in the treatment group were iv administered with Ginkgo Biloba Diterpene Lactone Glumine Injection on the basis of the control group, 5 mL added into normal saline 250 mL, once daily. Patients in two groups were treated for 14 d. After treatment, the clinical efficacy was evaluated, and BI, NIHSS and QOLISP scores, the serum adiponectin, neuron specific enolase and catalase levels, and adverse reactions in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control group was 81.67%, which was significantly lower than 95.08% in the treatment group, and there were differences between two groups (P < 0.05). After treatment, the BI, NIHSS and QOLISP scores in two groups were significantly increased (P < 0.05), and these scores in the treatment group were significantly higher than those in the control group (P < 0.05). After treatment, the serum adiponectin and catalase levels in two groups were significantly increased (P < 0.05), but the serum neuron specific enolase were significantly decreased (P < 0.05), and these indexes levels in the treatment group were significantly better than those in the control group (P < 0.05). During the treatment, the adverse reactions rate in the treatment group was 3.28%, which was significantly lower than 15.00% in control the group, with significant difference between two groups (P < 0.05). Conclusion Ginkgo Biloba Diterpene Lactone Glumine Injection combined with ozagrel has good clinical effect in treatment of acute cerebral infarction, and can significantly improve the living ability and quality of life, which has a certain clinical application value.
