目的 探讨渴乐宁胶囊联合达格列净治疗2型糖尿病的临床疗效。方法 选取2023年1月—2025年6月中国融通医疗健康集团邯郸二八五医院146例2型糖尿病患者，按照随机数字法分为对照组和治疗组，每组各73例。对照组餐前口服达格列净片，10 mg/次，1次/d。治疗组在对照组基础上联用渴乐宁胶囊，1.8 g/次，3次/d。所有患者均连续服药3个月。结束后，观察两组治疗效果，比较治疗前后空腹血糖（FBG）、餐后2 h血糖（2 h PG）、糖化血红蛋白（HbA1c）、总胆固醇（TC）、甘油三酯（TG）、低密度脂蛋白胆固醇（LDL‑C）、高密度脂蛋白胆固醇（HDL‑C）及空腹胰岛素（FINS）、胰岛β细胞功能指数（HOMA‑β）、胰岛素抵抗指数（HOMA‑IR）水平。结果 治疗组显效率和总有效率分别是65.75%、95.89%，均显著高于对照组的47.95%、84.93%，差异有统计学意义（P＜0.05）。治疗后，两组FBG、2 h PG及HbA1c均较治疗前显著降低（P＜0.05），治疗后组间对比显示，治疗组FBG、2 h PG及HbA1c水平均显著低于对照组（P＜0.05）。治疗后，两组TC、TG、LDL-C均较治疗前显著降低，HDL-C水平升高（P＜0.05）；治疗后，治疗组TC、TG、LDL-C低于对照组，HDL-C水平高于对照组（P＜0.05）。治疗后，两组FINS较前显著降低，HOMA-β升高，对照组HOMA-IR升高，治疗组HOMA-IR降低（P＜0.05）；治疗后，治疗组FINS、HOMA-IR低于对照组（P＜0.05）。结论 渴乐宁胶囊联合达格列净治疗2型糖尿病疗效确切，不仅能协同改善血糖及血脂代谢，还能显著改善胰岛素抵抗，值得临床推广应用。

Objective To investigate the clinical efficacy of Kelening Capsules combined with dapagliflozin in treatment of type 2 diabetes. Methods A total of 146 patients with type 2 diabetes from Handan 285 Hospital of China Rongtong Medical Health Group from January 2023 to June 2025 were selected. They were randomly divided into control group and treatment group, with 73 cases in each group. Patients in control group took Dapagliflozin Tablets orally before meals, 10 mg each time, once daily. Patients in treatment group were given Kelening Capsules in addition to control group regimen, 1.8 g each time, three times daily. All patients took the medication continuously for 3 months. After completion of the study, the therapeutic effects of two groups were observed, and the levels of fasting blood glucose (FBG), 2 h postprandial blood glucose (2 h PG), glycosylated hemoglobin (HbA1c), total cholesterol (TC), triglycerides (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), fasting insulin (FINS), insulin β-cell function index (HOMA-β), and insulin resistance index (HOMA-IR) before and after treatment were compared. Results The positive response rate and total effective rate in the treatment group were 65.75% and 95.89% respectively, which were significantly higher than those of the control group (47.95% and 84.93%), and the differences were statistically significant (P < 0.05). After treatment, both groups showed significant reductions in FBG, 2 h PG and HbA1c compared to before treatment (P < 0.05). After treatment, the levels of FBG, 2 h PG and HbA1c in treatment group were significantly lower than those in control group (P < 0.05). After treatment, both groups showed significant reductions in TC, TG and LDL-C, and increase in HDL-C (P < 0.05). After treatment, the TC, TG, and LDL-C levels in treatment group were lower than those in control group, and the HDL-C level was higher than that in control group (P < 0.05). After treatment, both groups showed significant decreases in FINS and an increase in HOMA-β, while HOMA-IR in control group increased and that in treatment group decreased (P < 0.05). After treatment, the FINS and HOMA-IR in treatment group were lower than those in control group (P < 0.05). Conclusion The combination of Kelening Capsules and dapagliflozin in treatment of type 2 diabetes has proven to be highly effective. It not only collaboratively improves blood sugar and lipid metabolism, but also significantly alleviates insulin resistance. It is worthy of clinical promotion and application.
