目的 观察复方甘菊利多卡因凝胶联合西帕依固龈液治疗种植体周围炎的效果。方法 选取沧州市人民医院2024年4月—2025年2月收治的92例种植体周围炎患者作为研究对象，采用随机数字表法将患者分为对照组（46例）和治疗组（46例）。对照组含漱西帕依固龈液2～3 min，3～5 mL/次，3～5次/d。治疗组在对照组基础上每次涂约0.5 cm复方甘菊利多卡因凝胶于疼痛或发生炎症的牙龈区，稍加按摩，3次/d。两组均治疗4周。比较两组的临床疗效、疼痛情况、生活质量、牙周指标、龈沟液炎症因子指标。结果 治疗组的总有效率97.83%高于对照组的总有效率82.61%（P＜0.05）；两组治疗后疼痛视觉模拟评分（VAS）评分下降，简明健康状况量表（SF-36）评分升高（P＜0.05）；且治疗组VAS评分低于对照组，SF-36评分高于对照组（P＜0.05）。两组治疗后菌斑指数（PLI）、出血指数（BI）、探诊深度（PD）、骨缺损深度（DD）下降（P＜0.05），且治疗组PLI、BI、PD、DD低于对照组（P＜0.05）。两组治疗后龈沟液超敏C反应蛋白（hs-CRP）、肿瘤坏死因子-α（TNF-α）、高迁移率族蛋白B1（HMGB1）、低氧诱导因子-1α（HIF-1α）水平下降（P＜0.05），且治疗组龈沟液hs-CRP、TNF-α、HMGB1、HIF-1α水平低于对照组（P＜0.05）。结论 复方甘菊利多卡因凝胶和西帕依固龈液联合治疗种植体周围炎的疗效显著，可减轻疼痛，改善生活质量，促进患者口腔健康，降低炎性反应水平。

Objective To observe the clinical efficacy of Compound Chamomile and Lidocaine Hydrochloride Gel combined with Xipayi Guyin Fluid in treatment of peri-implant inflammation. Methods 92 Patients with peri-implantitis inflammation admitted to Cangzhou People’s Hospital from April 2024 to February 2025 were selected as the research subjects. Using a random number table method, the patients were divided into control group (46 cases) and treatment group (46 cases). The control group performed oral rinses with Xipayi Guyin Fluid for 2 — 3 min, 3 — 5 mL per time, 3 — 5 times daily. The treatment group applied approximately 0.5 cm of Compound Chamomile and Lidocaine Hydrochloride Gel to the painful or inflamed gingival areas on the basis of the control group's regimen, followed by slight massage, 3 times daily. Two groups received treatment for 4 weeks. The clinical efficacy, pain levels, quality of life, periodontal indicators, and gingival crevicular fluid inflammatory factor indicators were compared between two groups. Results The total effective rate of the treatment group (97.83%) was higher than 82.61% of the control group (P < 0.05). After treatment, the visual analogue scale (VAS) scores for pain in two groups decreased, while the Short Form Health Status Scale (SF-36) scores were increased (P < 0.05). And the VAS score of the treatment group was lower than that of the control group, but the SF-36 score was higher than that of the control group (P < 0.05). After treatment, the plaque index (PLI), bleeding index (BI), probing depth (PD), and bone defect depth (DD) of two groups decreased (P < 0.05), and the PLI, BI, PD, and DD of the treatment group were lower than those of the control group (P < 0.05). After treatment, the levels of high-sensitivity C-reactive protein (hs-CRP), tumor necrosis factor-α (TNF-α), high mobility group protein B1 (HMGB1), and hypoxia inducible factor-1α (HIF-1α) in gingival crevicular fluid decreased in two groups (P < 0.05), and the levels of hs-CRP, TNF-α, HMGB1, and HIF-1α in gingival crevicular fluid in the treatment group were lower than those in the control group (P < 0.05). Conclusion The combination of Compound Chamomile and Lidocaine Hydrochloride Gel combined with Xipayi Guyin Fluid is effective in treatment of peri-implant inflammation, which can reduce pain, improve quality of life, promote patients' oral health and reduce the level of inflammatory reaction.
