目的 探讨复方石淋通胶囊联合枸橼酸钾治疗泌尿系结石的临床疗效。方法 选取2022年1月—2024年10月河北燕达医院收治的96例泌尿系结石患者，按随机数字表法分为对照组和治疗组，每组各48例。对照组口服枸橼酸钾缓释片，2片/次，3次/d。治疗组在此基础上联合使用复方石淋通胶囊口服治疗，6粒/次，3次/d。两组疗程4周。观察两组临床疗效，比较两组治疗前和治疗2周、4周时的疼痛视觉模拟评分法（VAS）评分，以及治疗前后结石数目、结石直径、尿红细胞计数及相关量表评分和血清C反应蛋白（CRP）水平。结果 治疗后，治疗组患者总有效率是95.83%，显著高于对照组的83.33%（P＜0.05）。治疗2、4周时，两组疼痛VAS评分均较治疗前显著降低（P＜0.05），且治疗组治疗后各时点均低于对照组同期（P＜0.05）。治疗后，两组结石数目和尿红细胞计数均显著减少，结石直径均显著缩小（P＜0.05）；治疗后均以治疗组改善更优（P＜0.05）。治疗后，两组症状自评量表（SCL-90）、阿森斯失眠量表（AIS）评分和血清CRP都低于同组治疗前，36项健康调查简表（SF-36）评分都显著增加（P＜0.05）；治疗后，治疗组SCL-90、AIS评分和血清CRP低于对照组，而SF-36评分高于对照组（P＜0.05）。结论 复方石淋通胶囊联合枸橼酸钾治疗泌尿系结石能有效减轻患者疼痛和血尿症状，促进结石缩小和排出，控制机体感染程度，对患者睡眠状况和身心综合健康状态有明显的改善效果。

Objective To explore the clinical efficacy of Compound Shilingtong Capsules combined with potassium citrate in treatment of urinary calculus. Methods 96 patients with urinary calculus admitted to Hebei Yanda Hospital from January 2022 to October 2024 were selected and divided into control group and treatment group according to random number table method, with 48 cases in each group. Patients in control group were po administered with Potassium Citrate Extended-release Tablets, 2 tablets/time, three times daily. Patients in treatment group were po administered with Compound Shilingtong Capsules on the basis of control group, 6 grains/time, three times daily. The treatment course for both groups was 4 weeks. The clinical efficacy of two groups was observed. The Visual Analogue Scale (VAS) scores for pain before treatment and at 2 weeks and 4 weeks of treatment were compared between two groups, and the number of stones, stone diameter, urine red blood cell count, related scale scores and serum CRP levels in two groups before and after treatment were compared. Results After treatment, the total effective rate of the treatment group was 95.83%, significantly higher than that of the control group (83.33%) (P < 0.05). At 2 and 4 weeks of treatment, the VAS scores for pain in both groups were significantly lower than those before treatment (P < 0.05), and the scores at each time point after treatment in treatment group were lower than those in control group at the same period (P < 0.05). After treatment, number of stones and urine red blood cell count in both groups decreased significantly, and stone diameters decreased significantly (P < 0.05). After treatment, the improvement was better in treatment group (P < 0.05). After treatment, scores of SCL-90, AIS, and serum CRP in both groups were lower than those before treatment in the same group, and scores of SF-36 increased significantly (P < 0.05). After treatment, SCL-90, AIS score and serum CRP of treatment group were lower than those of the control group, but SF-36 score was higher than that of control group (P < 0.05). Conclusion Compound Shilingtong Capsules combined with potassium citrate can effectively alleviate patients’ pain and hematuria symptoms in treatment of urinary calculus, and promote the shrinkage and excretion of calculus, control the degree of infection in the body, which has a significant improvement effect on patients’ sleep conditions and overall physical and mental health.
