目的 探讨采用解郁除烦胶囊与艾司西肽普兰协同治疗抑郁症的临床疗效。方法 选取2023年11月—2025年12月武汉市精神卫生中心诊治的86例抑郁症患者，按照随机数字表法分为对照组和治疗组，每组43例。对照组患者给予口服草酸艾司西酞普兰片，10 mg/次，1次/d。治疗组在对照组的基础上给予口服解郁除烦胶囊，2粒/次，3次/d。两组用药8周。观察两组的临床疗效，比较两组汉密尔顿抑郁量表（HAMD）评分、匹兹堡睡眠质量指数量表（PSQI）、简单健康状况调查表（SF-36）评分、血清神经递质因子和炎症因子水平。结果 治疗后，与对照组总有效率79.07%对比，治疗组总有效率95.35%明显升高（P＜0.05）。治疗后，两组PSQI评分、HAMD评分均较前显著下降，而SF-36评分升高（P＜0.05）；治疗后，与对照组对比，治疗组PSQI评分、HAMD评分均降低，治疗组SF-36评分升高（P＜0.05）。治疗后两组5-羟色胺（5-HT）、神经肽Y（NPY）、γ-氨基丁酸（GABA）水平均较同组治疗前显著升高（P＜0.05）；治疗后与对照组对比，治疗组5-HT、NPY、GABA水平均升高（P＜0.05）。治疗后，两组血清孤啡肽（OFQ）、同型半胱氨酸（Hcy）、白细胞介素1β（IL-1β）均较前显著下降，而脑源性神经生长因子（BDNF）显著升高（P＜0.05）；治疗后与对照组对比，治疗组OFQ、Hcy、IL-1β水平均较低，BDNF水平升高（P＜0.05）。结论 解郁除烦胶囊与艾司西肽普兰协同治疗抑郁症效果明显，可以减轻患者抑郁程度，提升睡眠及生活质量，调节神经递质因子和炎性因子水平，值得借鉴与应用。

Objective To investigate the therapeutic effect of Jieyu Chufan Capsules combined with escitalopram in treatment of depression. Methods A total of 86 patients with depression who were treated at Wuhan Mental Health Center from November 2023 to December 2025 were selected. They were divided into control group and treatment group according to random number table method, with 43 cases in each group. Patients in control group were given oral Escitalopram Oxalate Tablets, 10 mg each time, once daily. Patients in treatment group were given oral Jieyu Chufan Capsules, 2 capsules each time, 3 times daily on basis of control group. Both groups were administered the medication for 8 weeks. The clinical efficacy of two groups was observed, and Hamilton Depression Rating Scale (HAMD) scores, Pittsburgh Sleep Quality Index (PSQI), Simple Health Status Survey (SF-36) scores, and levels of serum neurotransmitter factors and inflammatory factors were compared between two groups. Results After treatment, compared with control group with a total effective rate of 79.07%, the total effective rate of treatment group was significantly higher at 95.35% (P < 0.05). After treatment, PSQI scores and HAMD scores of both groups decreased significantly compared to before, while the SF-36 scores increased (P < 0.05). After treatment, compared with control group, PSQI scores and HAMD scores of treatment group decreased, and SF-36 scores of treatment group increased (P < 0.05). After treatment, the levels of 5-hydroxytryptamine (5-HT), neuropeptide Y (NPY), and γ-aminobutyric acid (GABA) in both groups significantly increased compared to those before treatment in the same group (P < 0.05). After treatment, compared with control group, the levels of 5-HT, NPY, and GABA in treatment group increased (P < 0.05). After treatment, the serum hypocretin (OFQ), homocysteine (Hcy), and interleukin 1β (IL-1β) levels of both groups decreased significantly compared to before, while the brain-derived neurotrophic factor (BDNF) significantly increased (P < 0.05). After treatment, compared with control group, the levels of OFQ, Hcy, and IL-1β in treatment group were lower, and the BDNF level increased (P < 0.05). Conclusion The combination therapy of Jieyu Jufan Capsules and escitalopram shows significant effects in treatment of depression, and can alleviate the depression of patients, improve their sleep and quality of life, and regulate the levels of neurotransmitter factors and inflammatory factors. This treatment method is worthy of being studied and applied.
