目的 探讨白灵片联合他克莫司治疗儿童稳定期白癜风的临床疗效。方法 将2023年6月—2025年5月郑州大学附属儿童医院收治的80例稳定期白癜风患儿,经随机数字表法1∶1分配至治疗组与对照组。对照组患儿给予0.03%他克莫司软膏,2次/d。治疗组患儿在对照组基础上口服白灵片,3片/次,3次/d。两组连续治疗6个月。评估两组疗效,比较两组治疗前后皮损面积、色素积分、白细胞介素-17(IL-17)、干扰素-γ(IFN-γ)、肿瘤坏死因子-α(TNF-α)。结果 治疗后,治疗组总有效率(95.00%)较对照组(77.50%)显著提升17.5%(P<0.05)。治疗后,两组皮损面积及色素积分均显著改善(P<0.05),其中治疗组各指标改善幅度较对照组提升(P<0.05)。治疗后,两组炎症介质IL-17、IFN-γ和TNF-α显著下降(P<0.05),其中治疗组各指标降低幅度较对照组更显著(P<0.05)。结论 白灵片联合他克莫司治疗儿童稳定期白癜风疗效确切,能有效促进皮损复色,其作用机制可能与调节免疫,降低血清IL-17、IFN-γ、TNF-α等炎性因子水平有关。

Objective To explore the clinical efficacy of Bailing Tablets combined with tacrolimus in treatment of children with stable vitiligo. Methods From June 2023 to May 2025, 80 children with stable-stage vitiligo admitted to Zhengzhou University Affiliated Children’s Hospital were randomly assigned to treatment group and control group at a ratio of 1:1 using a random number table method. The control group was treated with 0.03% tacrolimus ointment applied topically twice daily. On the basis of the above treatment, the treatment group received oral Bailing Tablets at 3 tablets each time, three times daily. Both groups were treated continuously for 6 months. The clinical efficacy was evaluated. Lesion area, pigmentation score, and serum levels of interleukin-17 (IL-17), interferon-γ (IFN-γ) and tumor necrosis factor-α (TNF-α) were compared before and after treatment. Results After treatment, the total effective rate of the treatment group was 95.00%, which was higher than 77.50% of control group (P < 0.05). Both groups achieved significant improvements in lesion area and pigmentation score after treatment (P < 0.05), and the improvements were more prominent in the treatment group (P < 0.05). The levels of IL-17, IFN-γ and TNF-α were markedly decreased in both groups (P < 0.05), with more substantial reductions observed in the treatment group (P < 0.05). Conclusion The combination of Bailing Tablets and tacrolimus has a definite therapeutic effect on children with stable vitiligo and can effectively promote repigmentation of skin lesions. Its mechanism may be related to regulating immune function and reducing serum levels of inflammatory factors including IL-17, IFN-γ and TNF-α.
