目的 探讨热毒宁注射液联合注射用盐酸多西环素治疗儿童重症肺炎支原体肺炎的临床疗效。方法 选取2024年6月—2025年12月河北省沧州中西医结合医院收治的76例重症肺炎支原体肺炎患儿作为研究对象,按照随机数字表法将患儿分为对照组和治疗组,每组各38例。对照组静脉滴注注射用盐酸多西环素,2 mg/kg加入5%葡萄糖注射液100 mL,1次/12 h。治疗组在对照组基础上静脉滴注热毒宁注射液,0.5 mL/kg加入5%葡萄糖注射液100 mL,1次/d。两组均持续治疗7 d。比较两组的临床疗效、症状体征消失时间、动脉血气指标、血清炎症介质。结果 治疗后,治疗组的总有效率为94.74%,高于对照组的总有效率78.95%(P<0.05)。治疗组发热、乏力、咳嗽、咯痰、喘憋、肺部啰音消失时间均短于对照组(P<0.05)。治疗后,两组血氧饱和度(SaO2)、动脉血氧分压(pO2)、pH值均升高,、动脉血二氧化碳分压(pCO2)降低(P<0.05);治疗组SaO2、pO2、pH值高于对照组,pCO2低于对照组(P<0.05)。治疗后,两组血清超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、γ-干扰素(IFN-γ)、乳酸脱氢酶(LDH)水平均降低(P<0.05),治疗组血清hs-CRP、IL-6、IFN-γ、LDH水平低于对照组(P<0.05)。结论 热毒宁注射液联合注射用盐酸多西环素可提高儿童重症肺炎支原体肺炎疗效,快速缓解症状,改善血气,抑制炎症反应。

[Abstract]
Objective To investigate the clinical efficacy of Reduning Injection combined with Doxycycline Hydrochloride for injection in treatment of severe Mycoplasma pneumoniae pneumonia in children. Methods A total of 76 children with severe Mycoplasma pneumoniae pneumonia admitted to Cangzhou Integrated Traditional Chinese and Western Medicine Hospital of Hebei Province from June 2024 to December 2025 were selected as the research subjects. The children were divided into control group and treatment group using a random number table method, with 38 cases in each group. The control group received intravenous infusion of Doxycycline Hydrochloride for injection at a dose of 2 mg/kg and 100 mL of 5% glucose injection, once/12 hours. The treatment group received intravenous infusion of Reduning Injection in addition to the control group, at a dose of 0.5 mL/kg and 100 mL of 5% glucose injection, once daily. Two groups were treated continuously for 7 days. The clinical efficacy, disappearance time of symptoms and signs, arterial blood gas indicators, and serum inflammatory mediators were compared between two groups. Results After treatment, the total effective rate of the treatment group was 94.74%, which was higher than the total effective rate of 78.95% in the control group (P < 0.05). The disappearance time of fever, fatigue, cough, sputum, wheezing, and lung rales in the treatment group was shorter than that in the control group (P < 0.05). After treatment, the blood oxygen saturation (SaO2), arterial partial pressure of oxygen (pO2), and pH value in two groups increased, while arterial partial pressure of carbon dioxide (pCO2) decreased (P < 0.05). The SaO2, pO2, and pH values in the treatment group were higher than those in the control group, while pCO2 was lower than those in the control group (P < 0.05). After treatment, the serum levels of high-sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6), interferon gamma (IFN-γ), and lactate dehydrogenase (LDH) in two groups decreased (P < 0.05), and the serum levels of hs-CRP, IL-6, IFN-γ, and LDH in the treatment group were lower than those in the control group (P < 0.05). Conclusion Reduning Injection combined with Doxycycline Hydrochloride for injection can improve the clinical efficacy in treatment of severe Mycoplasma pneumoniae pneumonia in children, rapidly relieve clinical symptoms, improve blood gas, inhibit inflammatory response.
