目的 探究复方苁蓉益智胶囊联合艾地苯醌片治疗血管性痴呆的临床疗效。方法 选取2020年5月—2022年1月南阳市第二人民医院收治的120例血管性痴呆患者作为研究对象，将所有患者按照随机数字表法分为对照组和治疗组，每组各60例。对照组口服艾地苯醌片，1片/次，3次/d。治疗组在对照组治疗的基础上口服复方苁蓉益智胶囊，4粒/次，3次/d。两组患者均治疗8周。观察两组的临床疗效，比较两组的简易智能精神状态检查量表（MMSE）、Blessed行为量表（BBS）、痴呆程度量表（CDR）评分、血清指标水平。结果 治疗后，治疗组总有效率为93.33%，明显高于对照组的总有效率80.00%，组间比较差异具有显著性（P＜0.05）。治疗后，两组MMSE评分显著升高，BBS、CDR评分均显著降低（P＜0.05），且治疗组MMSE评分高于对照组，BBS、CDR评分低于对照组（P＜0.05）。治疗后，两组血清肿瘤坏死因子-α（TNF-α）、白细胞介素-6（IL-6）、丙二醛（MDA）水平均显著降低，血清超氧化物歧化酶（SOD）、成纤维细胞生长因子（bFGF）、脑源性神经营养因子（BDNF）水平均显著升高（P＜0.05），且治疗组血清TNF-α、IL-6、MDA水平明显低于对照组，血清SOD、bFGF、BDNF水平明显高于对照组（P＜0.05）。结论 复方苁蓉益智胶囊联合艾地苯醌片治疗血管性痴呆的效果显著，可有效改善患者精神、行为状况，减轻痴呆程度、机体炎症和机体氧化应激反应，调节神经修复因子水平，且安全性较高。

Objective To explore the clinical efficacy of Compound Congrong Yizhi Capsules combination with Idebenone Tablets in treatment of vascular dementia. Methods Patients (120 cases) with vascular dementia in Nanyang Second General Hospital from May 2020 to January 2022 were divided into control and treatment groups according to the random number table method, and each group had 60 cases. Patients in the control group were po administered with Idebenone Tablets, 1 tablet/time, three times daily. Patients in the treatment group were po administered with Compound Congrong Yizhi Capsules on the basis of the control group, 4 grains/time, three times daily. Patients in two groups were treated for 8 weeks. After treatment, the clinical efficacies were evaluated, and MMSE scores, BBS scores, CDR scores and serum indexes in two groups were compared. Results After treatment, the total effective rate of the treatment group was 93.33%, which was significantly higher than the total effective rate of the control group (80.00%), and there was a significant difference between two groups (P < 0.05). After treatment, MMSE scores of two groups were significantly increased, while BBS and CDR scores of two groups were significantly decreased (P < 0.05). MMSE scores of the treatment group were higher than those of the control group, while BBS and CDR scores were lower than those of the control group (P < 0.05). After treatment, the serum levels of TNF-α, IL-6, and MDA in two groups were significantly decreased, but the serum levels of SOD, bFGF, and BDNF in two groups were significantly increased (P < 0.05). The serum levels of TNF-α, IL-6, and MDA in the treatment group were significantly lower than those in the control group, but the serum levels of SOD, bFGF and BDNF in the treatment group were significantly higher than those in the control group (P < 0.05). Conclusion Yizhi Capsules combination with Idebenone Tablets has clinical curative effect in treatment of vascular dementia, can effectively improve the mental and behavioral status of patients, reduce the degree of dementia, inflammation and oxidative stress reaction of the body, regulate the level of nerve repair factors, with high safety.
