[摘要]
目的 研究银杏达莫注射液联合注射用鼠神经生长因子治疗重型急性颅脑损伤的临床疗效。方法 选取2016年8月—2020年3月焦作同仁医院接受治疗的75例重型急性颅脑损伤患者,将所有患者随机分为对照组(38例)和治疗组(37例)。对照组患者肌肉注射注射用鼠神经生长因子,30 μg加入到0.9%氯化钠注射液2 mL,1次/d。治疗组在对照组基础上静脉滴注银杏达莫注射液,10 mL加入0.9%氯化钠注射液500 mL中,2次/d。两组患者接受治疗时间为2周。观察两组患者临床疗效,比较两组的简易智力状态检查量表(MMSE)评分、格拉斯哥昏迷(GCS)评分、美国国立卫生研究院卒中量表(NIHSS)评分、血清肿瘤坏死因子-α(TNF-α)和白细胞介素-6(IL-6)、神经元特异性烯醇化酶(NSE)和S100β水平。结果 治疗后,治疗组总有效率94.74%显著高于对照组的78.38%(P<0.05)。治疗后,两组MMSE评分和GCS评分明显升高,NIHSS评分明显降低(P<0.05),且治疗组改善程度较大(P<0.05)。治疗后,两组血清TNF-α、IL-6、NSE和S100β蛋白水平显著降低(P<0.05);并且治疗组降低较明显(P<0.05)。结论 银杏达莫注射液联合注射用鼠神经生长因子治疗重型急性颅脑损伤具有较好的疗效,可改善患者认知功能,降低脑损伤程度,降低血清因子水平。

Objective To study the efficacy of Ginkgo Leaf Extract and Dipyridamole Injection combined with Mouse Nerve Growth Factor for injection in treatment of severe acute craniocerebral injury. Methods Patients (75 cases) with severe acute craniocerebral injury in Jiaozuo Tongren Hospital from August 2016 to March 2020 were randomly divided into the control group (38 cases) and the treatment group (37 cases). Patients in the control group were iv administered with Mouse Nerve Growth Factor for injection, 30 μg added into normal saline 2 mL, once daily. Patients in the treatment group were iv administered with Ginkgo Leaf Extract and Dipyridamole Injection on the basis of the control group, 10 mL added into normal saline 500 mL, twice daily. Patients in two groups were treated for 4 weeks. After treatment, the clinical efficacies were evaluated, and MMSE score, GCS score, NIHSS score, the serum levels of TNF-α, IL-6, NSE, and S100β in two groups were compared. Results After treatment, the total effective rate of the treatment group was 94.74%, which was significantly higher than 78.38% of the control group (P<0.05). After treatment, the MMSE score and the GCS score of two groups were significantly increased, but the NIHSS score of two groups was significantly decreased (P<0.05), and the improvement degree of the treatment group was better than that of the control group (P<0.05). After treatment, the serum levels of TNF-α, IL-6, NSE, and S100β protein in two groups were significantly decreased (P<0.05), and the observational indexes of the treatment group was significantly lower than those in the control group (P<0.05). Conclusion Ginkgo Leaf Extract and Dipyridamole Injection combined with Mouse Nerve Growth Factor for injection has clinical curative effect in treatment of severe acute craniocerebral injury, can improve the cognitive function of patients, reduce the degree of brain injury, and reduce the level of serum factors.
