[摘要]
目的 探讨芪明颗粒联合甲钴胺治疗糖尿病周围神经病变的临床疗效。方法 选取2017年1月—2018年7月内蒙古包钢医院收治的172例糖尿病周围神经病变患者作为研究对象,对照组口服甲钴胺片,1.5 mg/次,1次/d。治疗组在对照组基础上温水冲服芪明颗粒,4.5 g/次,3次/d。两组患者连续治疗3个月后统计疗效。观察两组的临床疗效,比较两组的血糖水平、感觉传导速度、多伦多周围神经病变评分系统(TCSS)评分、视觉模拟评分法(VAS)评分。结果 治疗后,对照组和治疗组的总有效率分别为72.09%、84.88%,两组比较差异有统计学意义(P<0.05)。治疗后,两组的正中神经、胫骨神经、下肢腓肠神经的运动传导速度(MCV)、感觉传导速度(SCV)均明显升高,同组治疗前后比较差异具有统计学意义(P<0.05);且治疗后,治疗组的正中神经、胫骨神经、下肢腓肠神经的MCV、SCV高于对照组,两组比较差异有统计学意义(P<0.05)。治疗后,两组的TCSS评分、VAS评分明显降低,同组治疗前后差异有统计学意义(P<0.05);且治疗后治疗组的TCSS评分、VAS评分低于对照组,两组差异有统计学意义(P<0.05)。结论 芪明颗粒联合甲钴胺治疗糖尿病周围神经病变具有较好的疗效,可改善患者临床症状,减轻疼痛,改善神经传导速度,具有一定的临床推广应用价值。

Objective To investigate the clinical effect of Qiming Granules combined with Mecobalamin Tablets in treatment of diabetic peripheral neuropathy. Methods Patients (172 cases) with diabetic peripheral neuropathy in Inner Mongolia Baogang Hospital from January 2014 to July 2018 were randomly divided into control and treatment groups, and each group had 86 cases. Patients in the control group were po administered with Mecobalamin Tablets, 1.5 mg/time, once daily. Patients in the treatment group were po administered with Qiming Granules on the basis of the control group, 4.5 g/time, three times daily. Patients in two groups were treated for 3 months. After treatment, the clinical efficacies were evaluated, and blood glucose levels, sensory conduction velocity, TCSS scores, and VAS scores in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 72.09% and 84.88%, respectively, and there was difference between two groups (P<0.05). After treatment, the MCV and SCV of median nerve, tibial nerve and lower limb sural nerve in two groups were significantly increased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly higher than those in the control group, with significant difference between two groups (P<0.05). After treatment, TCSS scores and VAS scores in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). Conclusion Qiming Granules combined with mecobalamin has clinical curative effect in treatment of diabetic peripheral neuropathy, can improve clinical symptoms, relieve pain, and improve nerve conduction velocity, which has a certain clinical application value.
