目的 探讨复方海蛇胶囊联合复方吡拉西坦脑蛋白水解物治疗老年痴呆症的临床疗效。方法 选取2014年4月—2018年4月在漯河市第六人民医院治疗老年痴呆症患者78例，根据入院的先后分为对照组（39例）和治疗组（39例）。对照组口服复方吡拉西坦脑蛋白水解物片，3片/次，2次/d；治疗组在对照组基础上口服复方海蛇胶囊，3粒/次，3次/d。两组患者均治疗4周。观察两组患者临床疗效，同时比较治疗前后两组患者MMSE、BDAE、MoCA、HDS和PSDI评分及血清白细胞介素-6（IL-6）、血管紧张素Ⅱ（AngⅡ）和降钙素基因相关肽（CGRP）水平。结果 治疗后，对照组临床有效率为79.49%，显著低于治疗组的94.87%，两组比较差异具有统计学意义（P<0.05）。治疗后，两组患者MMSE、BDAE、MoCA和HDS评分均显著升高（P<0.05），PSDI评分显著降低（P<0.05），且治疗组上述评分明显好于对照组（P<0.05）。治疗后，两组患者血清IL-6、AngⅡ水平均显著降低（P<0.05），CGRP表达显著增高（P<0.05），且治疗组IL-6、AngⅡ和CGRP水平明显好于对照组（P<0.05）。结论 复方海蛇胶囊联合复方吡拉西坦脑蛋白水解物片治疗老年痴呆症可有效改善认知功能，有利于语言功能和行为功能改善，具有一定的临床推广应用价值。

Objective To investigate the clinical efficacy of Compound Haishe Capsules combined with compound piracetam and cerebroprotein hydrolysate in treatment of senile dementia. Methods Patients (78 cases) with senile dementia in Luohe Sixth People's Hospital from April 2014 to April 2018 were into control (43 cases) and treatment (43 cases) groups according to admission order. Patients in the control group were po administered with Compound Piracetam and Cerebroprotein Hydrolysate Tablets, 3 tablets/time, twice daily. Patients in the treatment group were po administered with Compound Haishe Capsules on the basis of the control group, 3 grains/time, three times daily. Patients in two groups were treated for 4 weeks. After treatment, the clinical efficacy was evaluated, and the MMSE, BDAE, MoCA and HDS scores, and serum levels of IL-6, AngⅡand CGRP in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control group was 79.49%, which was significantly lower than 94.87% in the treatment group, and there were differences between two groups (P<0.05). After treatment, the MMSE, BDAE, MoCA and HDS scores in two groups were significantly increased (P<0.05), but PSDI scores were significantly decreased (P<0.05), and these scores in the treatment group were significantly better than those in the control group (P<0.05). After treatment, the serum IL-6 and AngⅡ levels in two groups were significantly decreased (P<0.05), but serum CGRP levels were significantly increased (P<0.05), and the IL-6, AngⅡ and CGRP levels in the treatment group were significantly better than those in the control group (P<0.05). Conclusion Compound Haishe Capsules combined with compound piracetam and cerebroprotein hydrolysate in treatment of senile dementia can effectively improve cognitive, language and behavior function, which has a certain clinical application value.
