目的 探讨复方丹参滴丸联合左卡尼汀治疗不稳定型心绞痛的临床效果。方法 选取2016年1月—2019年1月西藏自治区人民医院收治的126例不稳定型心绞痛患者，随机分为对照组和治疗组，每组各63例。对照组静脉滴注左卡尼汀注射液，3 g加入250 mL生理盐水中均匀混合后给药，1次/d。治疗组在对照组基础上口服复方丹参滴丸，10丸/次，3次/d。两组均连续治疗2周。观察两组的临床疗效，比较两组治疗前后心绞痛发作情况、心电图相关指标、血小板最大聚集率（MAR）、血栓前状态标志物和氧化应激指标的变化情况。结果 治疗后，对照组和治疗组的心绞痛总有效率分别是81.0%、93.7%；两组心电图总有效率分别是69.8%、85.7%，两组比较差异具有统计学意义（P<0.05）。治疗后，两组心绞痛发作次数、持续时间、硝酸甘油用量均较治疗前显著降低，同组治疗前后比较差异具有统计学意义（P<0.05）；治疗后，治疗组心绞痛发作次数、持续时间、硝酸甘油用量显著低于对照组，两组比较差异具有统计学意义（P<0.05）。治疗后，两组ST段压低数值总和（∑ST）、ST段下降导联数（NST）、心肌缺血总负荷（TIB）、血小板最大聚集率（MAR）、纤维蛋白原（FIB）、D-二聚体（D-D）水平均较治疗前显著减小，同组治疗前后比较差异具有统计学意义（P<0.05）；治疗后，治疗组心电图相关参数、MAR值及血栓前状态标志物水平显著低于对照组，两组比较差异具有统计学意义（P<0.05）。治疗后，两组血清超氧化物歧化酶（SOD）水平均显著上升，而丙二醛（MDA）、8异前列腺素F_2α（8-iso-PGF_2α）均显著下降，同组治疗前后比较差异具有统计学意义（P<0.05）；治疗后，治疗组SOD高于对照组，而MDA、8-iso-PGF_2α低于对照组，两组比较差异具有统计学意义（P<0.05）。结论 复方丹参滴丸联合左卡尼汀治疗不稳定型心绞痛的整体疗效确切，有助于稳定患者病情，改善心肌缺血状态，抑制血小板聚集，纠正血栓前状态，拮抗机体氧化应激，具有一定的临床推广应用价值。

Objective To investigate the clinical effect of Compound Danshen Dropping Pills combined with levocarnitine in treatment of unstable angina pectoris. Methods Patients (126 cases) with unstable angina pectoris in Tibet Autonomous Region People's Hospital from January 2016 to January 2019 were randomly divided into control (63 cases) and treatment (63 cases) groups. Patients in the control group were iv administered with Levocarnitine Injection, and 3 g was added to 250 mL normal saline and mixed evenly before administration, once daily. Patients in the treatment group were po administered with Compound Danshen Dropping Pills on the basis of the control group, 10 pills/time, three times daily. Patients in two groups were treated for 2 weeks. After treatment, the clinical efficacy was evaluated, the changes of angina pectoris attack, electrocardiogram related indicators, maximum platelet aggregation rate (MAR), prethrombotic status markers and oxidative stress indicators in two groups before and after treatment were compared. Results After treatment, the angina pectoris efficacy in the control and treatment groups were 81.0% and 93.7%, respectively, and electrocardiogram efficacy in two groups were 69.8% and 85.7%, respectively, and there were differences between two groups (P<0.05). After treatment, attacks frequency, duration, and nitroglycerin dosage in both groups were significantly reduced compared with those before treatment, and there were differences in the same group (P<0.05). After treatment, attacks frequency, duration, and nitroglycerin dosage in the treatment group were lower than those in the control group, and there were differences between two groups (P<0.05). After treatment, ∑ST, NST, TIB, MAR, FIB, and D-D in two groups were significantly decreased, and there were differences in the same group (P<0.05). After treatment, electrocardiogram parameters, MAR values and prethrombotic status markers in the treatment group were lower than those in the control group, and there were differences between two groups (P<0.05). After treatment, SOD in two groups was significantly increased, but MDA and 8-iso-PGF_2α were significantly decreased, and there were differences in the same group (P<0.05). After treatment, SOD in the treatment group was higher than that in the control group, but MDA and 8-iso-PGF_2α were lower than those in the control group, and there were differences between two groups (P<0.05). Conclusion Compound Danshen Dropping Pills combined with levocarnitine has significant effect in treatment of unstable angina pectoris, and is helpful to stabilize the condition of patients, and also can improve the myocardial ischemia state, inhibit platelet aggregation, correct the pre-thrombus state, and antagonize the oxidative stress of the body, which has a certain clinical application value.
