目的 探讨腰痛宁胶囊联合曲安奈德注射液治疗坐骨神经痛的临床疗效。方法 选取2016年8月—2017年12月兰考县中心医院收治的坐骨神经痛患者94例为研究对象，采用随机数字表法将所有患者分为对照组和治疗组，每组各47例。对照组痛点注射曲安奈德注射液，40 mg加入到2%利多卡因注射液2 mL中，1次/5 d，连续治疗3次。治疗组在对照组治疗的基础上口服腰痛宁胶囊，5粒/次，1次/d，连续治疗15 d。观察两组的临床疗效，比较两组的神经传导速度、炎性细胞因子水平和疼痛视觉模拟评分（VAS）。结果 治疗后，对照组和治疗组的总有效率分别为72.34%、89.36%，两组比较差异有统计学意义（P<0.05）。治疗后，两组腓肠神经感觉神经传导速度（SCV）、腓肠神经运动神经传导速度（MCV）、胫神经MCV均显著升高，同组治疗前后比较差异有统计学意义（P<0.05）；且治疗组神经传导速度明显高于对照组，两组比较差异具有统计学意义（P<0.05）。治疗后，两组肿瘤坏死因子-α（TNF-α）、白细胞介素-6（IL-6）、单核细胞趋化蛋白-1（MCP-1）水平均明显降低，同组治疗前后比较差异有统计学意义（P<0.05）；且治疗组炎性细胞因子水平明显低于对照组，两组比较差异具有统计学意义（P<0.05）。治疗后，两组VAS评分均显著降低，同组治疗前后比较差异有统计学意义（P<0.05）；且治疗组VAS评分明显低于对照组，两组比较差异具有统计学意义（P<0.05）。结论 腰痛宁胶囊联合曲安奈德注射液治疗坐骨神经痛具有较好的临床疗效，可减轻疼痛程度，提高神经传导速度，降低炎症反应，具有一定的临床推广应用价值。

Objective To investigate the clinical efficacy of Yaotongning Capsules combined with Triamcinolone Acetonide Injection in treatment of sciatica. Methods Patients (94 cases) with sciatica in Lankao Central Hospital from August 2016 to December 2017 were randomly divided into control and treatment groups, and each group had 47 cases. Patients in the control group were trigger point injection administered with Triamcinolone Acetonide Injection, 40 mg added into 2% Lidocaine Hydrochloride Injection 2 mL, once every 5 days, treated for 3 times. Patients in the treatment group were po administered with Yaotongning Capsules on the basis of the control group, 5 grains/time, once daily, treated for 15 d. After treatment, the clinical efficacies were evaluated, and nerve conduction velocity, inflammatory cytokine level and VAS scores in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 72.34% and 89.36%, respectively, and there was difference between two groups (P < 0.05). After treatment, the SCV of gastrocnemius nerve and the MCV of gastrocnemius nerve and tibial nerve in two groups were significantly increased, and the difference was statistically significant in the same group (P < 0.05). And the nerve conduction velocities in the treatment group were significantly higher than those in the control group, with significant difference between two groups (P < 0.05). After treatment, the levels of TNF-α, IL-6, and MCP-1 in two groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). And the inflammatory cytokine levels in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P < 0.05). After treatment, the VAS scores in two groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). And the VAS scores in the treatment group was significantly lower than that in the control group, with significant difference between two groups (P < 0.05). Conclusion Yaotongning Capsules combined with Triamcinolone Acetonide Injection has clinical curative effect in treatment of sciatica, can relieve pain, increase nerve conduction velocity, and reduce inflammation reaction, which has a certain clinical application value.
