目的 探讨痰热清注射液联合注射用阿昔洛韦治疗老年反复呼吸道感染的临床疗效。方法 选取2016年1月-2017年1月在四川大学华西医院进行治疗的老年反复呼吸道感染患者105例为研究对象，所有患者均随机分为对照组（55例）和治疗组（50例）。对照组静脉滴注注射用阿昔洛韦，5 mg/kg，3次/d，隔8 h滴注1次，每次滴注时间在1 h以上。治疗组在对照组治疗的基础上静脉滴注痰热清注射液，20 mL加入到5%葡萄糖注射液500 mL中，1次/d。两组患者均持续治疗28 d。观察两组的临床疗效，比较两组的临床症状和免疫功能。结果 治疗后，对照组和治疗组的总有效率分别为76.36%、98.00%，两组比较差异有统计学意义（P<0.05）。治疗后，治疗组症状好转时间、体温恢复时间、单次感染时间、再次复发次数均显著低于对照组，两组比较差异具有统计学意义（P<0.05）。治疗后，治疗组免疫球蛋白M（IgM）、免疫球蛋白G（IgG）、免疫球蛋白A（IgA）水平均显著降低，同组治疗前后比较差异具有统计学意义（P<0.05）；且治疗组这些免疫因子水平明显低于对照组治疗同期，两组比较差异具有统计学意义（P<0.05）。结论 痰热清注射液联合注射用阿昔洛韦治疗老年反复呼吸道感染具有较好的临床疗效，可改善患者临床症状，提高免疫水平，复发率低，具有一定的临床推广应用价值。

Objective To investigate the clinical effect of Tanreqing Injection combined with Aciclovir for injection in treatment of recurrent respiratory tract infections in the elderly. Methods Elderly patients (105 cases) with respiratory tract infections in West China Hospital of Sichuan University from January 2016 to January 2017 were randomly divided into the control group (55 cases) and the treatment group (50 cases). Patients in the control group were iv administered with Aciclovir for injection, 5 mg/kg, three times daily, each time interval was 8 h, and the time of infusion was above 1 h. Patients in the treatment group were iv administered with Tanreqing Injection on the basis of the control group, 20 mL added into 5% glucose solution 500 mL, once daily. Patients in two groups were treated for 28 d. After treatment, the clinical efficacies were evaluated, and clinical symptoms and immune function in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 76.36% and 98.00%, respectively, and there was difference between two groups (P<0.05). After treatment, symptoms improvement time, body temperature recovery time, single infection time, and relapse frequency in the treatment group were significantly lower than those in the control groups, and there was difference between two groups (P<0.05). After treatment, the levels of immunoglobulin M (IgM), IgG, and IgA in the treatment group were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the immune factors levels in the treatment group were significantly lower than those in the control group in the same period, with significant difference between two groups (P<0.05). Conclusion Tanreqing Injection combined with Aciclovir for injection has clinical curative effect in treatment of recurrent respiratory tract infections in the elderly, can improve the clinical symptoms of patients, and increase the immune levels of patients, with the low recurrence rate, which has a certain clinical application value.
