目的 探讨橘红痰咳液联合乙酰半胱氨酸治疗慢性阻塞性肺疾病（COPD）患者的临床疗效。方法 选取2016年10月-2017年10月在濮阳市安阳地区医院进行治疗的COPD患者82例，随机将患者分为对照组（41例）和治疗组（41例）。对照组口服溶解后的乙酰半胱氨酸泡腾片，0.6 mg/次，2次/d；治疗组在对照组基础上口服橘红痰咳液，20 mL/次，3次/d。两组患者均经过12周治疗。评价两组患者临床疗效，同时比较治疗前后两组患者动脉血气分析指标、肺功能指标、血清细胞因子水平和氧化应激水平。结果 治疗后，对照组临床有效率为80.49%，显著低于治疗组的95.12%，两组比较差异具有统计学意义（P<0.05）。治疗后，两组氧分压（pO₂）和血氧饱和度（SaO₂）水平及一秒用力呼气容积（FEV₁）、用力肺活量（FVC）和最大呼气流量（PEF）均显著升高（P<0.05），二氧化碳分压（pCO2）水平显著下降（P<0.05），且治疗组上述动脉血气和肺功能指标改善后水平明显优于对照组，两组比较差异具有统计学意义（P<0.05）。治疗后，两组血清IL-6、髓样细胞触发受体-1（sTREM-1）、单核细胞趋化因子-1（MCP-1）和丙二醛（MDA）水平均显著降低（P<0.05），γ-干扰素（IFN-γ）、超氧化物歧化酶（SOD）和谷胱甘肽过氧化物酶（GPX）水平显著增加（P<0.05）。同时，治疗后治疗组上述血清细胞因子和氧化应激水平明显优于对照组，两组比较差异具有统计学意义（P<0.05）。结论 橘红痰咳液联合乙酰半胱氨酸治疗COPD，有利于患者肺功能和动脉血气指标改善，并降低机体炎症和氧化应激水平。

Objective To evaluate the clinical efficacy of Juhong Tanke Liquid combined with acetylcysteine in treatment of chronic obstructive pulmonary disease. Methods Patients (82 cases) with chronic obstructive pulmonary disease in Anyang District Hospital from October 2016 to October 2017 were randomly divided into control (41 cases) and treatment (41 cases) groups. Patients in the control group were po administered with Acetylcysteine Effervescent Tablets dissolved in water, 0.6 mg/time, twice daily. Patients in the treatment group were po administered with Juhong Tanke Liquid on the basis of the control group, 20 mL/time, three times daily. Patients in two groups were treated for 12 weeks. After treatment, the clinical efficacy was evaluated, and the arterial blood gas indexes, the pulmonary function, the serum cytokine levels, the levels of oxidative stress in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control group was 80.49%, which was significantly lower than 95.12% in the treatment group, and there were differences between two groups (P<0.05). After treatment, the pO₂, SaO₂, FEV₁, FVC, and PEF levels in two groups were significantly increased (P<0.05), but the pCO2 levels were significantly decreased (P<0.05), at the same time the arterial blood gas and pulmonary function indexes in the treatment group after treatment were significantly better than those in the control group, with significant difference between two groups (P<0.05). After treatment, the serum IL-6, sTREM-1, MCP-1 and MDA levels in two groups were significantly decreased (P<0.05), but IFN-γ, SOD and GPX levels were significantly increased (P<0.05), at the same time, the serum cytokine levels and the levels of oxidative stress in the treatment group after treatment were significantly better than those in the control group, with significant difference between two groups (P<0.05). Conclusion Juhong Tanke Liquid combined with acetylcysteine in treatment of chronic obstructive pulmonary disease can effectively improve the pulmonary function and arterial blood gas, reduce the inflammatory reaction and oxidative stress level.
