目的 探讨鸦胆子油乳注射液联合丝裂霉素和氟尿嘧啶治疗肝癌的临床疗效。方法 选取2012年1月—2014年7月在咸阳市第一人民医院治疗的肝癌患者102例，根据用药的差别分为对照组（51例）和治疗组（51例）。对照组静脉注射注射用丝裂霉素，4 mg加入250 mL生理盐水，1次/周；对照组患者同时静脉注射氟尿嘧啶注射液，500 mg/m²加入250 mL生理盐水，1次/d，第1～14天给药；治疗组在对照组的基础上静脉注射鸦胆子油乳注射液，20 mL加入250 mL生理盐水，1次/d，第1～14天给药。3周为1个疗程，两组均治疗2个疗程。观察两组患者临床疗效，比较治疗前后两组患者血清学指标、QLQ-C30评分和生存率差别。结果 治疗后，对照组客观缓解率（ORR）和临床获益率（CBR）分别为54.90%、76.47%，均显著低于治疗组的70.59%、94.12%，两组比较差异具有统计学意义（P<0.05）。治疗后，两组血清血管内皮生长因子（VEGF）、缺氧诱导因子-1A（HIF-1A）、结缔组织生长因子（CTGF）、基质金属蛋白酶-2（MMP-2）水平均明显降低（P<0.05）；且治疗组比对照组降低更显著（P<0.05）。治疗后，两组患者躯体、认知、情绪、角色和社会功能评分均明显升高（P<0.05）；且治疗组比对照组升高更明显（P<0.05）。治疗组1、2、3年生存率分别为88.24%、78.43%、64.71%，均分别高于同时期对照组患者（62.75%、56.86%、41.18%），两组比较差异具有统计学意义（P<0.05）。结论 鸦胆子油乳注射液联合丝裂霉素和氟尿嘧啶治疗肝癌可有效改善患者生活质量，提高生存率，有效抑制肿瘤新生血管生成，具有一定的临床推广应用价值。

Objective To investigate the clinical effect of Yadanzi Oil Emulsion Injection combined with mitomycin and fluorouracil in treatment of liver cancer. Methods Patients (102 cases) with liver cancer in the First People's Hospital of Xianyang City from January 2012 to July 2014 were randomly divided into control (51 cases) and treatment (51 cases) groups. Patients in the control group were iv administered with Mitomycin for injection, 4 mg added into normal saline 250 mL, once weekly. At the same time, they were iv administered with Fluorouracil Injection, 500 mg/m² added into normal saline 250 mL, once daily for the first two weeks. Patients in the treatment group were iv administered with Yadanzi Oil Emulsion Injection on the basis of the control group, 20 mg added into normal saline 250 mL, once daily for the first two weeks. Patients in two groups were treated for 2 courses, and a course was 3 weeks. After treatment, the clinical efficacy was evaluated, and the differences of serological indexes, QLQ-C30 scores and survival rate in two groups before and after treatment were compared. Results After treatment, the ORR and CBR in the control group were 54.90% and 76.47%, which were significantly lower than 70.59% and 94.12% in the treatment group, respectively, and there were differences between two groups (P < 0.05). After treatment, the VEGF, HIF-1A, CTGF, and MMP-2 levels in two groups were significantly decreased (P < 0.05). And the serological indexes levels in the treatment group were significantly lower than those in the control group (P < 0.05). After treatment, the physical, cognition, mood, role and social function scores in two groups were significantly increased (P < 0.05). And the QLQ-C30 scores in the treatment group were significantly higher than those in the control group (P < 0.05). The 1, 2, and 3 years survival rates in the treatment group were respectively 88.24%, 78.43% and 64.71%, which were significantly higher than 62.75%, 56.86%, and 41.18% in the control group, respectively, and there were differences between two groups (P < 0.05). Conclusion Yadanzi Oil Emulsion Injection combined with mitomycin and fluorouracil can effectively improve the quality of life and the survival rate in treatment of liver cancer, and inhibit tumor angiogenesis, which has a certain clinical application value.
