目的 观察吸入用布地奈德混悬液联合注射用氢化可的松琥珀酸钠治疗小儿喘息性支气管肺炎的临床疗效。方法 选取2015年2月-2017年3月南阳市中心医院收治的喘息性支气管肺炎患儿70例为研究对象，采用随机数表法将所有患儿分为对照组和治疗组，每组各35例。对照组雾化吸入吸入用布地奈德混悬液，1 mg/次，2次/d。治疗组在对照组基础上静脉滴注注射用氢化可的松琥珀酸钠，5~10 mg/kg，1次/d。两组患儿均连续治疗1周。观察两组的临床疗效，比较两组的肺功能、血清炎症因子和免疫球蛋白E（IgE）水平。结果 治疗后，对照组和治疗组的总有效率分别为77.14%、94.29%，两组比较差异有统计学意义（P<0.05）。治疗后，两组第一秒用力呼气量（FEV1）、用力肺活量（FVC）、FEV1/FVC均显著升高，同组治疗前后比较差异有统计学意义（P<0.05）；且治疗组这些观察指标明显高于对照组，两组比较差异具有统计学意义（P<0.05）。治疗后，两组血清肿瘤坏死因子（TNF-α）、白细胞介素-6（IL-6）、C-反应蛋白（CRP）、IgE水平均明显下降，同组治疗前后比较差异有统计学意义（P<0.05）；且治疗组这些观察指标明显低于对照组，两组比较差异具有统计学意义（P<0.05）。结论 吸入用布地奈德混悬液联合注射用氢化可的松琥珀酸钠治疗小儿喘息性支气管肺炎的疗效较好，能明显改善患儿肺功能，调节炎症因子水平，安全性较好，具有一定的临床推广应用价值。

Objective To observe the clinical effects of Budesonide Suspension for inhalation combined with Hydrocortisone Sodium Succinate for injection in treatment of asthmatic bronchopneumonia in children. Methods Children (70 cases) with asthmatic bronchopneumonia in Nanyang Central Hospital from February 2015 to March 2017 were randomly divided into control and treatment groups, and each group had 35 cases. Children in the control group were aerosol inhalation administered with Budesonide Suspension for inhalation, 1 mg/time, twice daily. Children in the treatment group were iv administered with Hydrocortisone Sodium Succinate for injection on the basis of the control group, 5-10 mg/kg, once daily. Patients in two groups were treated for 1 week. After treatment, the clinical efficacies were evaluated, and lung function, serum inflammatory factors, and IgE levels in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 77.14% and 94.29%, respectively, and there was difference between two groups (P<0.05). After treatment, FEV1, FVC, and FEV1/FVC in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). After treatment, the levels of TNF-α, IL-6, CRP, and IgE in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). Conclusion Budesonide Suspension for inhalation combined with Hydrocortisone Sodium Succinate for injection has clinical curative effect in treatment of asthmatic bronchopneumonia in children, can significantly improve lung function, and regulate inflammatory factor levels, with good safety, which has a certain clinical application value.
