目的 探讨银杏达莫注射液联合胸腺法新治疗自发性腹膜炎的临床疗效。方法 选取2015年2月-2017年6月费县人民医院收治的92例自发性腹膜炎患者，随机分为对照组和治疗组，每组各46例。对照组皮下注射注射用胸腺法新，1.6mg/次，2次/d。治疗组在对照组治疗基础上静脉滴注银杏达莫注射液，25 mL/次，1次/d。两组均连续治疗2周。观察两组的临床疗效，比较两组临床症状消失时间，比较两组治疗前后肝功能和炎症因子水平的变化情况。结果 治疗后，对照组和治疗组的总有效率分别为71.74%、91.30%，两组比较差异有统计学意义（P<0.05）。治疗组发热、腹痛、腹胀、反跳痛消失时间均显著短于对照组，两组比较差异有统计学意义（P<0.05）。治疗后，两组血清白蛋白（ALB）、白细胞介素-10（IL-10）均显著增加，谷氨酸氨基转移酶（ALT）、肿瘤坏死因子-α（TNF-α）、IL-6显著降低，治疗组总胆红素（TBIL）显著降低，同组治疗前后差异有统计学意义（P<0.05）；治疗后，治疗组TBIL、ALT、TNF-α、IL-6显著低于对照组，IL-10水平高于对照组，两组比较差异有统计学意义（P<0.05）。结论 银杏达莫注射液联合胸腺法新治疗自发性腹膜炎疗效显著，可缩短临床症状消失时间，改善肝功能，促进炎症介质的吸收，具有一定的临床推广应用价值。

Objective To investigate the clinical efficacy of Ginkgo Leaf Extract and Dipyridamole Injection combined with thymalfasin in treatment of spontaneous peritonitis. Methods Patients (82 cases) with spontaneous peritonitis in Feixian County People's Hospital from February 2015 to June 2017 were randomly divided into control (46 cases) and treatment (46 cases) groups. Patients in the control group were iv administered with Thymalfasin for Injection, 1.6 mg/time, twice daily. Patients in the treatment group were iv administered with Ginkgo Leaf Extract and Dipyridamole Injection on the basis of the control group, 25 mL/time, once daily. Patients in two groups were treated for 2 weeks. After treatment, the clinical efficacy was evaluated, and clinical symptoms disappeared time, liver function levels and inflammatory factors in two groups were compared. Results After treatment, the clinical efficacy in the control and treatment groups were 71.74% and 91.30%, respectively, and there was difference between two groups (P<0.05). The disappeared time of fever, abdominal pain, abdominal distension and rebound pain in the treatment group were significantly shorter than those in the treatment group, and there was difference between two groups (P<0.05). After treatment, ALB and IL-10 in two groups were significantly increased, but ALT, TNF-α, and IL-6 in two groups were significantly decreased. TBIL in the treatment group was significantly decreased, and the difference was statistically significant in the same group (P<0.05). After treatment, TBIL, ALT, TNF-α, and IL-6 in the treatment group were lower than those in the control group, but IL-10 was higher than that in the control group, and there was difference between two groups (P<0.05). Conclusion Ginkgo Leaf Extract and Dipyridamole Injection combined with thymalfasin has a significant effect in treatment of spontaneous peritonitis, and can shorten clinical symptoms disappeared time, also can improve liver function and promote absorption of inflammatory mediators, which has a certain clinical application value.
