目的 探讨复方肾炎片联合来氟米特和泼尼松治疗狼疮性肾炎（LN）的临床疗效。方法 选取2015年1月-2015年6月在海口市人民医院接受治疗的LN患者84例，根据治疗方案的差别分为对照组和治疗组，每组各42例。对照组口服醋酸泼尼松片，1 mg/（kg·d），2个月后逐渐减至10 mg/d；同时口服来氟米特片，20 mg/d。治疗组在对照基础上口服复方肾炎片，3片/次，3次/d。两组患者均连续治疗6个月。观察两组的临床疗效，比较治疗前后两组患者各项血肌酐（Scr）、24 h尿蛋白定量（24 h Upro）、红细胞（RBC）、β₂-微球蛋白（β₂-MG）、血浆白蛋白（Alb）、红细胞沉降率（ESR）和补体C3情况以及SLEDAI积分变化。结果 治疗后，对照组和治疗组的总有效率分别为71.43%、90.48%，两组比较差异具有统计学意义（P<0.05）。治疗后，两组患者Scr、24 h Upro、RBC计数、β₂-MG、ESR水平均较治疗前显著降低，Alb和C3水平显著升高，同组治疗前后差异具有统计学意义（P<0.05）；且治疗组这些观察指标的改善程度优于对照组，两组比较差异具有统计学意义（P<0.05）。治疗后，两组患者SLEDAI积分均明显降低（P<0.05），治疗组患者SLEDAI积分比对照组降低更显著（P<0.05）。对照组和治疗组的不良反应发生率分别为26.19%、9.52%，两组比较差异有统计学意义（P<0.05）。结论 复方肾炎片联合来氟米特、泼尼松治疗LN效果显著，可明显提高临床缓解率，且不良反应发生率低，具有一定的临床推广应用价值。

Objective To investigate the clinical effect of Compound Shenyan Tablets combined with leflunomide and prednisone in treatment of lupus nephritis. Methods Patients (84 cases) with lupus nephritis in Haikou People's Hospital from January 2015 to June 2015 were divided into control and treatment groups according to different treatments, and each group had 42 cases. The patients in the control group were po administered with Prednisone Acetate Tablets, 1 mg/(kg·d), the dosage was gradually reduced to 10 mg/d after two months. And they were po administered with Leflunomide Tablets, 20 mg/d. The patients in the treatment group were po administered with Compound Shenyan Tablets on the basis of the control group, 3 tablets/time, three times daily. The patients in two groups were treated for six months. After treatment, the efficacy was evaluated, and the changes of Scr, 24 h Upro, RBC, β₂-MG, Alb, ESR, and C3 levels, and SLEDAI scores in two groups before and after treatment were compared. Results After treatment, the efficacy in the treatment group (90.48%) was higher than that (71.43%) of the control group, and there were differences between two groups (P<0.05). After treatment, levels of Scr, 24 h Upro, RBC, β₂-MG, and ESR in two groups were significantly decreased, but levels of Alb and C3 were significantly increased, and the difference was statistically significant in the same group (P<0.05). After treatment, the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05). After treatment, SLEDAI scores in two groups were significantly decreased (P<0.05), which were significantly better in the treatment group than those in the control group (P<0.05). The incidence of adverse reactions in the control and treatment groups were 26.19% and 9.52%, respectively, and there was difference between two groups (P<0.05). Conclusion Compound Shenyan Tablets combined with leflunomide and prednisone has clinical curative effect in treatment of lupus nephritis, can significantly improve the clinical remission rate with a low incidence of adverse reactions, which has a certain clinical application value.
