目的 探讨榄香烯注射液联合培美曲塞和奈达铂治疗晚期肺癌的临床疗效。方法 选取2014年2月—2016年2月在通山县人民医院接受治疗的晚期肺癌患者86例，按照治疗方法的不同分成对照组（43例）和治疗组（43例）。两组患者均给予必要的基础治疗。对照组静脉滴注注射用培美曲塞二钠，0.5 mg/m2；同时静脉滴注注射用奈达铂，75 mg/m2，每21 d给药1次，21 d为一个疗程。治疗组在对照组基础静脉滴注榄香烯注射液，0.6 g加入5%葡萄糖溶液500 mL，1次/d，21 d为一个疗程，休息1周后进行下一疗程。两组患者均连续治疗2个疗程。观察两组的临床疗效，比较两组治疗前后生存质量、免疫功能和血清基质金属蛋白酶（MMP）水平的改善情况。结果 治疗后，对照组和治疗组客观缓解率（ORR）分别为55.81%和76.74%，两组临床获益率（CBR）分别为65.12%和83.72%，两组近期临床疗效比较差异具有统计学意义（P<0.05）。治疗后，对照组和治疗组生存质量改善率分别为81.40%和95.35%，两组患者改善率比较差异具有统计学意义（P<0.05）。治疗后，两组患者CD³⁺、CD⁴⁺、CD⁴⁺/CD⁸⁺均明显升高，同组比较差异具有统计学意义（P<0.05），且治疗组升高的更明显，两组比较差异具有统计学意义（P<0.05）。治疗后，两组患者血清MMP-2和MMP-9水平均较同组治疗前明显降低（P<0.05），且治疗组降低的更显著（P<0.05）。结论榄香烯注射液联合培美曲塞和奈达铂治疗晚期肺癌具有很好的临床疗效，可以提高患者免疫能力及改善生活质量，具有一定的临床应用推广价值。

Objective To explore the clinical effect of Elemene Injection combined with pemetrexed and nedaplatin in treatment of advanced lung cancer. Methods Patients (86 cases) with advanced lung cancer in the People's Hospital of Tongshan from February 2014 to February 2016 were divided into control group (43 cases) and treatment group (43 cases) according to the different treatments. All patients were given necessary basic treatment. The patients in the control group were iv administered with Pemetrexed Disodium for injection, 0.5 mg/m²; And they were iv administered with Nedaplatin for injection, 75 mg/m², once every 21 d, and 21 d for a course of treatment. The patients in the treatment group were iv administered with Elemene Injection on the basis of control group, 0.6 g added into 5% glucose solution 500 mL, once daily, 21 d for a course of treatment, and were repeated the next course of treatment after 1 week's rest. All patients were treated for two courses of treatment. After treatment, the clinical short-term effect was evaluated, and the improvement of survival quality, immune function, and serum MMP levels in two groups were compared. Results After treatment, ORR in the control group and treatment group were 55.81% and 76.74%, respectively, and CBR in two groups were 65.12% and 83.72%, and there were differences between two groups (P<0.05). After treatment, the improvement rates of survival quality in the control group and treatment group were 81.40% and 95.35%, respectively, and there were differences between two groups (P<0.05). After treatment, CD³⁺, CD⁴⁺, and CD⁴⁺/CD⁸⁺ in two groups were significantly increased, and the difference was statistically significant in the same group (P<0.05), and the treatment group increased more significantly, with significant difference between two groups (P<0.05). After treatment, serum MMP-9 and MMP-2 levels in two groups were significantly decreased (P<0.05), and the treatment group decreased more significantly (P<0.05). Conclusion Elemene injection combined with pemetrexed and nedaplatin has a good clinical curative effect in treatment of advanced lung cancer, can enhance the immune function and improve the survival quality, which has a certain clinical application value.
