目的 探讨化痰平喘片联合氨茶碱缓释片治疗慢性阻塞性肺气肿的临床疗效。方法 选取2014年5月-2016年1月人民解放军海军机关门诊部呼吸内科收治的慢性阻塞性肺气肿患者86例,根据治疗方案的差别将所有患者分为对照组和治疗组,每组各43例。对照组口服氨茶碱缓释片,0.2g/次,3次/d。治疗组在对照组治疗基础上口服化痰平喘片,6片/次,2次/d。两组患者均连续治疗2周。观察两组的临床疗效,同时比较治疗前后氧分压(pO₂)、血氧饱和度(SpO₂)、二氧化碳分压(pCO₂)、一秒用力呼气容积(FEV₁)、用力肺活量(FVC)、FEV₁/FVC、白细胞介素4(IL-4)、IL-8、基质金属蛋白酶9(MMP-9)和肿瘤坏死因子α(TNF-α)的变化情况。结果 治疗后,对照组和治疗组的总有效率分别为83.72%、93.02%,两组比较差异有统计学意义(P<0.05)。治疗后,两组患者pO₂、SpO₂、FEV₁、FVC、FEV₁/FVC均较治疗前显著升高,pCO₂、IL-4、IL-8、MMP-9、TNF-α较治疗前显著降低,同组治疗前后差异有统计学意义(P<0.05);且治疗组这些观察指标的改善程度优于对照组,两组比较差异有统计学意义(P<0.05)。结论 化痰平喘片联合氨茶碱缓释片治疗慢性阻塞性肺气肿具有较好的临床疗效,可明显改善患者的肺功能,同时还可降低血清炎性因子水平,具有一定的临床推广应用价值。

Objective To investigate the clinical effect of Huatan Pingchuan Tablets combined with Aminophylline Sustained-release Tablets in treatment of chronic obstructive pulmonary emphysema. Methods Patients (86 cases) with chronic obstructive pulmonary emphysema in Naval Authority Clinics of the Chinese People's Liberation Army from May 2014 to January 2016 were enrolled in this study. According to the different treatment plans, patients were divided into control group (43 cases) and treatment group (43 cases). The patients in the control group were po administered with Aminophylline Sustained-release Tablets, 0.2 g/time, three times daily. The patients in the treatment group were po administered with Huatan Pingchuan Tablets on the basis of control group, 6 tablets/time, twice daily. The patients in two groups were treated for 2 weeks. After treatment, the clinical efficacies were evaluated, and the changes of pO₂, SpO₂, pCO₂, FEV₁, FVC, FEV₁/FVC, IL-4, IL-8, MMP-9, and TNF-α in two groups before and after treatment were compared. Results After treatment, the efficacies in the control and treatment groups were 83.72% and 93.02%, respectively, and there were differences between two groups (P < 0.05). After treatment, pO₂, SpO₂, FEV₁, FVC, and FEV₁/FVC in two groups were significantly increased, and pCO₂, IL-4, IL-8, MMP-9, and TNF-α in two groups were significantly decreased, and the difference was statistically significant in the same group (P < 0.05). After treatment, the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P < 0.05). Conclusion Huatan Pingchuan Tablets combined with Aminophylline Sustained-release Tablets has clinical curative effect in treatment of chronic obstructive pulmonary emphysema, and can obviously improve the lung function, and can also reduce the levels of inflammatory cytokines, which has a certain clinical application value.
