目的 分析天津市中心妇产科医院抗菌药物处方的使用情况，指导临床合理用药。方法 抽取2012—2013年天津市中心妇产科医院门急诊抗菌药物处方，并分析抗菌药物的使用及不合格处方情况。结果 2012年门急诊抗菌药物的使用率分别为8.28%、13.94%；2013年分别为7.98%、9.66%。2012、2013年被点评的抗菌药物处方分别为5 791、4 783张，其中处方不合格率分别为28.6%、17.6%。不合格处方主要为临床诊断指证不明确和选药不恰当，其中2012、2013年临床诊断指证不明确处方分别为1 379、575张，在不合格处方中不明确率分别为83.2%、68.3%；选药不恰当处方分别为277、267张，在不合格处方中不当率分别为16.8%、31.7%。结论 天津市中心妇产科医院的抗菌药物使用基本合理，但仍存在需要改正的问题，以后还需继续加大力度宣传抗菌药物的合理应用，保证患者的用药安全。

Objective To analyze the usage of antibacterial prescriptions in Tianjin Central Hospital of Gynecology Obstetrics, in order to guide the clinical rational drug use. Methods The antibacterial prescriptions in Tianjin Central Hospital of Gynecology Obstetrics from 2012 to 2013 were selected, and the usage of antibacterials and unqualified prescriptions were analyzed.Results The usage rates of antibacterials in outpatient and emergency department were 8.28% and 13.94% in 2012; 7.98% and 9.66% in 2013, respectively. The numbers of antibacterial prescriptions reviewed in 2012 and 2013 were 5 791 and 4 783, and the ratios of unqualified prescriptions were 28.6% and 17.6%, respectively. The unqualified prescriptions were mainly that the evidence of clinical diagnosis was not clear and the choice of antibacterials was inappropriate. The numbers of antibacterials in 2012 and 2013 whose evidence of clinical diagnosis was not clear were 1 379 and 575, and the ratios were 83.2% and 68.3%, respectively, in the unqualified prescriptions. The numbers of antibacterials whose choice of antibacterials was inappropriate were 277 and 267, and the ratios were 16.8% and 31.7%, respectively, in the unqualified prescriptions. Conclusion The utilization of antibacterials in Tianjin Central Hospital of Gynecology Obstetrics is basically rational. However, there are still problems and shortcomings. So it needs to promote the reasonable application of antibacterials to further ensure the safety of medication in patients.
