[摘要]
目的 观察注射用丹参多酚酸对脑梗死患者认知功能改善及对血清淀粉样蛋白A(SAA)及胱抑素C(CysC)水平的影响。方法 选取2018年1月-2018年12月保定市第一中心医院治疗的急性脑梗死患者100例作为研究对象,依照随机数字表将患者对照组(50例)和观察组(50例)。对照组予以常规治疗。观察组在对照组治疗基础上静点注射用丹参多酚酸,0.13 g予以0.9%氯化钠注射液250 mL稀释后,1次/d。两组疗程均为14 d。观察两组患者的临床疗效,同时比较两组治疗前后的蒙特利尔认知评估量表(MoCA)评分、改良长谷川痴呆量表(HDS-R)评分,以及血清淀粉样蛋白A(SSA)和胱抑素C(CysC)水平。结果 治疗后,观察组患者的总有效率为90.00%,显著高于对照组的74.00%,两组比较差异具有统计学意义(P<0.05)。治疗14、30 d后,两组MoCA评分均较治疗前均显著提高(P<0.05);观察组MoCA评分显著高于同期对照组(P<0.05)。治疗14 d后,两组HDS-R评分均较治疗前均显著提高(P<0.05),观察组HDS-R评分高于对照组,但无显著性差异;治疗30 d后,两组HDS-R评分均较治疗前均显著升高(P<0.05),且观察组HDS-R评分显著高于对照组(P<0.05)。治疗14、30 d后,两组SAA和血清CysC水平较治疗前均显著下降,同组治疗前后比较差异具有统计学意义(P<0.05),且观察组SAA和血清CysC水平显著低于对照组,两组比较差异具有统计学意义(P<0.05)。结论 注射用丹参多酚酸可明显改善急性脑梗死患者认知功能障碍,降低SAA和CysC水平,对远期预后的改善有积极的治疗效果。

Objective To observe the effect of Salvianolic Acids for Injection on cognitive function improvement and serum SAA and CysC levels in patients with cerebral infarction. Methods A total of 100 patients with acute cerebral infarction in the Baoding NO.1 Central Hospital from January 2018 to December 2018 were divided into control group (50 cases) and observation group (50 cases). Patients in the control group received routine treatment. Patients in the observation group were in administered with Salvianolic Acids for Injection on the basis of control group, 0.13 g was diluted with 0.9% sodium chloride injection 250 mL, once daily. The course of treatment in both groups was 14 d. The clinical efficacy in two groups was observed, and the MoCA score, HDSR score, and the levels of SSA and CysC before and after treatment in two groups were compared. Results After treatment, the total effective rate in the observation group was 90.00%, which was significantly higher than 74.00% in the control group, and the difference between two groups was statistically significant (P<0.05). After 14 and 30 days of treatment, MoCA scores in two groups were significantly increased (P<0.05). MoCA score in the observation group was significantly higher than that in the control group (P<0.05). After 14 days of treatment, THE HDS-R scores of two groups were significantly improved (P<0.05). The HDS-R scores in the observation group were higher than those of the control group, but there was no significant difference. After 30 days of treatment, HDS-R scores in two groups were significantly higher than those before treatment (P<0.05), and the HDS-R score in the observation group was significantly higher than that in the control group (P<0.05). After 14 and 30 days of treatment, serum SAA and CysC levels in two groups were significantly decreased compared with that before treatment, and the difference in the same group before and after treatment was statistically significant (P<0.05). Moreover, SAA and CysC levels in the observation group were significantly lower than those in the control group, with statistically significant differences between the two groups (P<0.05). Conclusion Salvianolic Acids for Injection can significantly improve cognitive dysfunction in patients with acute cerebral infarction, reduce SAA and CysC levels, and has a positive therapeutic effect on long-term prognosis.
