[摘要]
目的 探讨长春胺缓释胶囊联合小牛血去蛋白提取物注射液治疗前部缺血性视神经病变的安全性与有效性。方法 选择郑州市第二人民医院2015年10月—2017年10月收治的前部缺血性视神经病变患者135眼,随机分成对照组(67眼)和治疗组(68眼)。对照组患者静脉滴注小牛血去蛋白提取物注射液,20 mL/次,1次/d;治疗组患者在对照组基础上口服长春胺缓释胶囊,1粒/次,2次/d。两组患者均连续治疗4周。观察两组患者临床疗效,同时比较治疗前后两组患者平均光敏感度和平均视力水平、平均盘周神经纤维层和视网膜神经节细胞复合体厚度。结果 治疗后,对照组和治疗组临床有效率分别为82.09%和95.59%,两组患者比较差异均具有统计学意义(P<0.05)。治疗后,两组患者平均光敏感度和平均视力水平均显著提高(P<0.05),且治疗组患者平均光敏感度和平均视力水平明显高于对照组(P<0.05)。治疗后,两组患者的平均盘周神经纤维层和视网膜神经节细胞复合体厚度均显著减小(P<0.05),且治疗组患者平均盘周神经纤维层和视网膜神经节细胞复合体厚度明显小于对照组(P<0.05)。结论 长春胺缓释胶囊联合小牛血去蛋白提取物注射液治疗前部缺血性视神经病变疗效显著、安全性好,具有一定的临床推广应用价值。

Objective To investigate the safety and efficacy of Vincamine Sustained Release Capsules combined with deproteinized calf blood extractives in treatment of anterior ischemic optic neuropathy. Methods Patients (135 eyes) with anterior ischemic optic neuropathy in Zhengzhou Second Hospital from October 2015 to October 2017 were randomly divided into control (67 eyes) and treatment (68 eyes) groups. Patients in the control group were iv administered with Deproteinized Calf Blood Extractives Injection, 25 mL/time, once daily. Patients in the treatment group were po administered with Vincamine Sustained Release Capsules on the basis of the control group, 1 grain/time, twice daily. Patients in two groups were treated for 4 weeks. After treatment, the clinical efficacy was evaluated, and the average of photosensitivity, vision levels, thickness of the perirenal nerve fiber layer, and retinal ganglion cell complex in two groups before and after treatment were compared. Results After treatment, the clinical efficacy and in the control and treatment groups was 82.09% and 95.59%, respectively, and there were differences between two groups (P<0.05). After treatment, the average of photosensitivity and vision levels in two groups were significantly increased (P<0.05), and these functions in the treatment group were significantly higher than that in the control group (P<0.05). After treatment, the average of thickness of the perirenal nerve fiber layer and retinal ganglion cell complex in two groups were significantly reduced (P<0.05), and which in the treatment group were significantly smaller than that in the control group (P<0.05). Conclusion Vincamine Sustained Release Capsules combined with deproteinized calf blood extractives has significant efficacy in treatment of anterior ischemic optic neuropathy with high safety, which has a certain clinical application value.
