目的 探讨桉柠蒎肠溶软胶囊联合两性霉素B治疗真菌性鼻窦炎的临床疗效。方法 选取2021年7月—2023年7月青岛市城阳区人民医院收治的112例真菌性鼻窦炎患者，随机分为对照组（56例）和治疗组（56例）。对照组患者静脉滴注两性霉素B，25 mg加入生理盐水100 mL，每2天1次。治疗组在对照组的基础上口服桉柠蒎肠溶软胶囊，0.3 g/次，3次/d。两组治疗14 d。观察两组患者临床疗效，比较治疗前后两组患者症状缓解时间，生活质量量表（SF-36），血清因子干扰素-γ（INF-γ）、树突状细胞相关C型凝集素-1（Dectin-1）、肿瘤坏死因子-α（TNF-α）和白细胞介素-17（IL-17），及不良反应情况。结果 治疗后，治疗组临床总有效率为98.21%，明显高于对照组总有效率（82.14%，P＜0.05）。治疗后，治疗组症状缓解时间均明显早于对照组（P＜0.05）。治疗后，两组患者内容生理功能（RP）、社会功能（SF）、情感职能（RE）、总体健康（CH）评分比治疗前均明显升高（P＜0.05），且治疗组评分明显高于对照组（P＜0.05）。治疗后，两组血清因子IL-17、TNF-α、Dectin-1明显降低，而INF-γ水平明显升高（P＜0.05），且治疗组血清因子水平明显好于对照组（P＜0.05）。治疗后，对照组不良反应发生率为14.20%，明显高于治疗组（3.57%，P＜0.05）。结论 桉柠蒎肠溶软胶囊联合两性霉素B治疗真菌性鼻窦炎可较好改善临床症状，降低鼻窦炎症反应状态，有效提升生活质量。

Objective To explore the treatment effect of Eucalyptol, Limonene and Pinene Enteric Soft Capsules combined with amphotericin B in treatment of fungal sinusitis. Methods Patients (112 cases) with fungal sinusitis in Qingdao Chengyang People’s Hospital from July 2021 to July 2023 were randomly divided into control (56 cases) and treatment (56 cases) group. Patients in the control group were iv administered with Amphotericin B for injection, 25 mg added into normal saline 100 mL, once every 2 d. Patients in the treatment group were po administered with Eucalyptol, Limonene and Pinene Enteric Soft Capsules on the basis of the control group, 0.3 g/time, three times daily. Patients in two groups were treated for 14 d. After treatment, the clinical evaluations were evaluated, the symptom relief time, SF-36 scores, the levels of serological IL-17, TNF-α, Dectin-1 and INF-γ, and adverse reactions in two groups before and after treatment were compared. Results After treatment, the clinical effective rate of the treatment group was 98.21%, which was significantly higher than that of the control group (82.14%, P < 0.05). After treatment, the time of symptom relief in the treatment group was significantly earlier than that in the control group (P < 0.05). After treatment, the scores of RP, SF, RE and CH in two groups were significantly higher than those before treatment (P < 0.05), and the scores in the treatment group were significantly higher than those in the control group (P < 0.05). After treatment, the levels of serum factors IL-17, TNF-α and Dectin-1 in two groups were significantly decreased, while the level of INF- γ was significantly increased (P < 0.05), and the level of serum factors in the treatment group was significantly better than that in the control group (P < 0.05). After treatment, the incidence of adverse reactions in the control group was 14.20%, which was significantly higher than that in the treatment group (3.57%, P< 0.05). Conclusion Amphotericin B combined with Eucalyptol, Limonene and Pinene Enteric Soft Capsules in treatment of fungal sinusitis can improve clinical symptoms, reduce the state of nasal sinus inflammation and improve the quality of life.
