目的 探讨八宝丹胶囊联合哌拉西林他唑巴坦治疗急性胆囊炎的临床研究。方法 选取2021年1月—2021年12月南开大学附属医院（天津市第四医院）外科治疗的80例急性胆囊炎患者，使用随机数字表法将所有患者分为对照组和治疗组，每组各40例。对照组静脉滴注注射用哌拉西林钠他唑巴坦钠，2.25 g/次，加入0.9%氯化钠250 mL中稀释后静脉滴注，2次/d。治疗组在对照组的基础上口服八宝丹胶囊，0.6 g/次，2次/d。两组用药7 d。观察两组的临床疗效，比较两组症状好转时间、血常规指标、肝功能指标和血清学因子水平。结果 治疗后，治疗组总有效率是97.50%，显著高于对照组的77.50%（P＜0.05）。经治疗，治疗组出现发热、恶心呕吐、右上腹压痛、口苦等症状好转时间均短于对照组（P＜0.05）。治疗后，两组白细胞（WBC）、中性粒细胞（NEUT）、天门冬酸氨基转移酶（AST）、丙氨酸氨基转移酶（ALT）均较治疗前显著降低（P＜0.05），治疗后，治疗组患者血常规及肝功能指标改善优于对照组（P＜0.05）。治疗后，两组患者白细胞介素-6（IL-6）、血清总胆汁酸（TBA）、肿瘤坏死因子-α（TNF-α）、C反应蛋白（CRP）水平均较治疗前显著降低（P＜0.05），治疗后，治疗组血清学指标水平低于对照组（P＜0.05）。结论 八宝丹胶囊联合哌拉西林他唑巴坦治疗疗效确切，能使症状显著改善，对肝脏损伤程度减轻，能降低体内炎症反应，且药物安全有效，值得临床推广。

Objective To investigate the clinical effect of Babaodan Capsules combined with piperacillin sodium and tazobactam sodium in treatment of acute cholecystitis. Methods A total of 80 patients with acute cholecystitis who received surgical treatment in Affiliated Hospital of Nankai University (Tianjin Fourth Hospital) from January 2021 to December 2021 were selected. All patients were divided into control group and treatment group by random number table method, with 40 cases in each group. Patients in the control group were iv administered with Piperacillin Sodium and Tazobactam Sodium for injection, 2.25 g/time, diluted into 250 mL 0.9% sodium chloride, intravenous infusion, twice daily. Patients in the treatment group were po administered with Babaodan Capsules on the basis of the control group, 0.6 g/time, twice daily. Both groups took the medicine for 7 d. The clinical efficacy of the two groups were observed, and the symptom improvement time, blood routine indexes, liver function indexes, and serological factors levels of the two groups were compared. Results After treatment, the total effective rate of the treatment group was 97.50%, significantly higher than that of the control group (77.50%) (P < 0.05). After treatment, the improvement time of fever, nausea and vomiting, right upper abdominal tenderness, bitter mouth in the treatment group was shorter than that in the control group (P < 0.05). After treatment, the indexes of white blood cell (WBC), neutrophil (NEUT), aspartate aminotransferase (AST), and alanine aminotransferase (ALT) in two groups were significantly decreased compared with those before treatment (P < 0.05). After treatment, the improvement of blood routine and liver function indexes in treatment group was better than that in control group (P < 0.05). After treatment, the levels of interleukin-6 (IL-6), serum total bile acid (TBA), tumor necrosis factor α (TNF-α), and C-reactive protein (CRP) in two groups were significantly decreased compared with those before treatment (P < 0.05). The serum indexes in treatment group were lower than those in control group (P < 0.05).Conclusion Babaodan Capsules combined with piperacillin sodium and tazobactam sodium has curative effect in treatment of acute cholecystitis, and can significantly improve the symptoms, reduce the degree of liver injury and the inflammatory response safely and effectively, which is worthy of clinical promotion.
