目的 探讨复方牙痛酊联合西吡氯铵含漱液治疗牙龈炎的临床疗效。方法 选取2018年2月-2019年1月在南京中医药大学附属南京医院（南京市第二医院）就诊的94例（患牙102颗）牙龈炎患者作为研究对象，将全部患者按照随机数字表法分为对照组（47例，患牙50颗）和治疗组（47例，患牙52颗）。对照组给予西吡氯铵含漱液，15 mL/次，5次/d，含漱时间15 min，含漱前后30 min不进食。治疗组在对照组基础上给予复方牙痛酊，治疗前使用生理盐水和过氧化氢进行牙面清洁，吹干后，采用小棉签浸润复方牙痛酊后涂抹于患处，3次/d。两组患者连续治疗15 d。观察两组的临床疗效，比较两组的牙周指标、龈沟液炎症因子水平、视觉模拟评分法（VAS）评分。结果 治疗后，对照组和治疗组的总有效率分别为80.00%、94.23%，两组比较差异有统计学意义（P<0.05）。治疗后，两组的牙周袋深度（PD）、牙龈指数（GI）、龈沟出血指数（SBI）、菌斑指数（PLI）均显著降低，同组治疗前后比较差异有统计学意义（P<0.05）；且治疗后治疗组的牙周指标明显低于对照组，差异有统计学意义（P<0.05）。治疗后，两组的龈沟液肿瘤坏死因子-α（TNF-α）、前列腺素E2（PGE2）、白细胞介素-8（IL-8）水平均明显降低，同组治疗前后比较差异有统计学意义（P<0.05）；且治疗后治疗组的龈沟液炎症因子水平明显低于对照组，差异有统计学意义（P<0.05）。治疗后，两组VAS评分显著降低，同组治疗前后比较差异有统计学意义（P<0.05）；且治疗组VAS评分明显低于对照组，差异有统计学意义（P<0.05）。结论 复方牙痛酊联合西吡氯铵含漱液治疗牙龈炎具有较好的临床疗效，可减轻疼痛程度，改善牙周指标，降低炎症反应，具有一定的临床推广应用价值。

Objective To investigate the clinical efficacy of Compound Yatong Tincture combined with Cetylpyridinium Chloride Gargle in treatment of gingivitis. Methods Patients (94 cases, 102 diseased teeth) with gingivitis in Nanjing Hospital Affiliated to Nanjing University of Traditional Chinese Medicine (Nanjing Second Hospital) from February 2018 to January 2019 were randomly divided into the control group (47 cases, 50 diseased teeth) and the treatment group (47 cases, 52 diseased teeth). Patients in the control group were given Cetylpyridinium Chloride Gargle, 15 mL/time, five times daily, gargled for 15 min, no food for 30 min before and after gargling. Patients in the treatment group were given Compound Yatong Tincture on the basis of the control group, before treatment, normal saline and hydrogen peroxide were used to clean the teeth. After drying, cotton swabs were used to infiltrate the Compound Yatong Tincture and then applied to the affected area, three times daily. Patients in two groups were treated for 15 weeks. After treatment, the clinical efficacies were evaluated, and periodontal indexes, inflammatory factors, and VAS scores in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 80.00% and 94.23%, respectively, and there was difference between two groups (P<0.05). After treatment, PD, GI, SBI, and PLI in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the periodontal indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). After treatment, the levels of TNF-α, PGE2, and IL-8 in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the inflammatory factors levels in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). After treatment, VAS scores in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the VAS score in the treatment group were significantly lower than that in the control group, with significant difference between two groups (P<0.05). Conclusion Compound Yatong Tincture combined with Cetylpyridinium Chloride Gargle has clinical curative effect in treatment of gingivitis, can alleviate pain, improve periodontal indicators and reduce inflammation, which has a certain clinical application value.
