目的 探讨小牛血清去蛋白注射液联合奥扎格雷钠氯化钠注射液治疗老年急性脑梗死的临床疗效。方法 选取2014年10月-2016年1月昆山市第一人民医院收治的老年急性脑梗死患者81例，随机分为对照组（40例）和研究组（41例）。对照组静脉滴注奥扎格雷钠氯化钠注射液，80 mg加入到250 mL生理盐水中，1次/d。研究组在对照组基础上静脉滴注小牛血清去蛋白注射液，30 mL加入到250 mL生理盐水中，1次/d。两组患者均治疗15 d。观察两组的临床疗效，比较两组的美国国立卫生研究院卒中量表（NIHSS）评分、同型半胱氨酸（Hcy）、S100B蛋白（S100B）和基质金属蛋白酶-8（MMP-8）水平以及改良Rankin量表（mRS）评分。结果 治疗后，对照组和治疗组的总有效率分别为77.50%、92.68%，两组总有效率比较差异有统计学意义（P<0.05）。治疗后，两组NIHSS评分均明显下降，同组治疗前后比较差异有统计学意义（P<0.05）；且治疗组NIHSS评分的下降程度明显优于对照组，两组比较差异具有统计学意义（P<0.05）。治疗后，两组Hcy、S100B、MMP-8水平均显著降低，同组治疗前后比较差异具有统计学意义（P<0.05）；且治疗组这些观察指标的下降程度明显优于对照组，两组比较差异具有统计学意义（P<0.05）。治疗后90 d，治疗组的mRS评分明显小于对照组，两组比较差异具有统计学意义（P<0.05）。结论 小牛血清去蛋白注射液联合奥扎格雷钠氯化钠注射液治疗老年脑梗死具有较好的临床疗效，能改善脑神经功能及预后，调节Hcy、S100B和MMP-8的水平，具有一定的临床推广应用价值。

Objective To investigate the clinical effect of Deproteinised Calf Blood Serum Injection combined with Sodium Ozagrel and Sodium Chloride Injection in treatment of senile acute cerebral infarction. Methods Patients (81 cases) with senile acute cerebral infarction in the First People's Hospital of Kunshan from October 2014 to January 2016 were randomly divided into control group (40 cases) and treatment group (41 cases). Patients in the control group were iv administered with Sodium Ozagrel and Sodium Chloride Injection, 80 mg added into normal saline 250 mL, once daily. Patients in the treatment group were iv administered with Deproteinised Calf Blood Serum Injection on the basis of the control group, 30 mL added into normal saline 250 mL, once daily. Patients in two groups were treated for 15 d. After treatment, the clinical efficacies were evaluated, and levels of Hcy, S100B, and MMP-8, NIHSS scores, and mRS scores in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 77.50% and 92.68%, respectively, and there was difference between two groups (P<0.05). After treatment, the NIHSS scores in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the NIHSS score in the treatment group was significantly lower than that in the control group, with significant difference between two groups (P<0.05). After treatment, the levels of Hcy, S100B, and MMP-8 in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). After treatment for 90 d, the mRS scores in the treatment group were lower than those in the control group, and there was difference between two groups (P<0.05). Conclusion Deproteinised Calf Blood Serum Injection combined with Sodium Ozagrel and Sodium Chloride Injection has clinical curative effect in treatment of senile acute cerebral infarction, can improve brain function and prognosis, and regulate the levels of Hcy, S100B, and MMP-8, which has a certain clinical application value.
