目的 探究坎地沙坦治疗老年原发性高血压及其并发症的临床疗效。方法 选取2011年1月—2014年1月冀中能源峰峰集团有限公司总医院收治的原发性高血压患者150例, 随机分为对照组和治疗组, 每组75例。对照组患者给予常规治疗, 治疗组患者在此基础上口服坎地沙坦酯片, 1片/次, 1次/d。两组患者均连续治疗24周。比较两组的临床疗效, 同时检测两组患者左心室舒张末期内径(LVDd)、左心室舒张末期后壁厚度(LVPWT)、左心室舒张末期室间隔厚度(VST)、最大管腔内膜界面与中层外膜界面之间的距离(IMT值)的变化。结果 对照组和治疗组总有效率分别为81.33%、96.00%, 两组比较差异有统计学意义(P<0.05)。治疗后, 两组收缩压和舒张压均明显下降, 同组治疗前后差异有统计学意义(P<0.05), 且治疗组的下降程度大于对照组, 两组比较差异有统计学意义(P<0.05)。治疗后, 治疗组LVDd、LVPWT、VST、IMT值均较治疗前降低, 差异有统计学意义(P<0.05), 但对照组治疗前后这些指标变化无统计学意义;治疗后, 治疗组LVDd、LVPWT、VST、IMT值低于对照组, 两组比较差异有统计学意义(P<0.05)。结论 坎地沙坦治疗老年原发性高血压及其并发症具有较好临床疗效, 可显著降低患者血压, 同时还可改善动脉粥样硬化和左心室肥大情况, 值得临床推广应用。

Objective To investigate the clinical effect of candesartan in treatment of elderly primary hypertension and its complications. Methods The patients with primary hypertension (150 cases) of General Hospital of Hebei Central Energy Fengfeng Group Co., LTD from January 2011 to January 2014 were randomly divided into control and treatment groups, and each group had 75 cases. The patients in the control group were given conventional therapy. The patients in the treatment group were po administered with Candesartan Cilexetil Tablets at the basis of control group, 1 tablet/time, once daily. The patients in two groups were treated for 24 weeks. After treatment, the efficacy was evaluated, while the changes of LVDd, LVPWT, VST, and IMT were detected. Results The efficacies in the control and treatment groups were 81.33% and 96.00%, respectively, and there were differences between the two groups (P < 0.05). After treatment, systolic blood pressure and diastolic blood pressure in two groups significantly decreased, and the difference was statistically significant in the same group before and after treatment (P < 0.05). Systolic blood pressure and diastolic blood pressure in treatment group decreased more than those in the control group, and there were differences between two groups (P < 0.05). After treatment, LVDd, LVPWT, VST, and IMT in treatment group significantly decreased, and the difference was statistically significant in the same group before and after treatment (P < 0.05). But there was no difference before and after treatment about these indexes in the control group. After treatment, the values of LVDd, LVPWT, VST, and IMT in the treatment group were lower than those in the control group, and there were differences between two groups (P < 0.05). Conclusion Candesartan has the good clinical effect in treatment of elderly primary hypertension and its complications, and can significantly decrease the blood pressure and improve the condition of atherosclerosis and left ventricular hypertrophy, which is worth clinical promotion.
